摘要
目的分析应用一期前路病灶清除钛网异体骨植骨钛板内固定术治疗颈椎结核的临床效果。方法2010年—2016年行前路病灶切除椎间植骨钢板内固定术治疗下颈椎结核患者25例。结果患者术后1.5~2年获得骨性愈合,平均融合时间20个月。3例Frank分级B级和5例Frank分级C级患者术后恢复达D级,其余皆完全恢复。术后随访时间12~36个月,平均随访时间24.3个月。术前VAS评分平均为8分,术后6个月随访为1.5分。颈椎后凸畸形矫正良好,术后后凸角0°~5°,较术前(0°~30°)明显减少,随访期间矫正角度无明显丢失,无植骨及内固定松动、脱落、折断等相关并发症。结论一期前路病灶清除钛网异体骨植骨内固定是治疗下颈椎结核的一种安全有效的手术方式。
Objective To explore the clinical efficacy of one-stage anterior debridement titanium cage fusion with allograft bone and internal fixation for cervical vertebra tuberculosis.Methods Between2010 and 2016,25 patients with lower cervical spine tuberculosis（C3-C7）underwent One-stage anterior debridement titanium cage fusion with allograft bone and internal fixation were selected as the research subjects.Results After one year,stable bone union was observed in all cases and the average time required for fusion was 6.8 months.Patients′clinical symptoms and the neurological dysfunctions had been improved in different degrees.In all cases,seventeen of them combined with neurological deficits improved to Frankel E,three and five cases improved from Frankel B and C to Frankel D.All patients underwent a follow-up for 12-36 months,with average of 24.3 months.The average preoperative VAS scale was 8（range 6-10）,and decreased to an average of 1.5（range 0-3）at the last follow-up.The kyphosis was corrected well,and the kyphosis Cobb angle improved from the preoperative average of 15.4°（range-0°-30°）to a postoperative average of-4.8°（range -12° to 4°）and there was no significant correction loss during the follow-up period.During the follow-up period,there were no grafts or instrumentation-related stabilization problems.Conclusion In the treatment of cervical vertebra tuberculosis,One-stage anterior debridement titanium cage fusion with allograft bone is a safe and effective surgical method.
