摘要
目的探索临床药师在门诊开展临床药学工作的模式,为深入临床药学工作提供思路。方法通过分析门诊慢性病患者处方的合理性,临床药师参与医保管理,审核门诊慢性病患者治疗方案的合理性及控制医保费用。结果临床药师审核门诊慢性病患者治疗方案模式创新不仅节约药品费用,控制医保费用增长,促进门诊处方合理率增加,同时也是临床药师参与慢性病团队管理的创新模式。结论慢病药学管理是药师今后开展工作的重要内容和平台,可以补充内科医疗服务,帮助医疗团队管理慢性病,同时也在保障患者用药安全、控制医药费用增长中发挥越来越重要的作用。
Objective To explore the mode of clinical pharmacy work of clinical pharmacists in outpatient de- partment, so as to provide ideas for further improving clinical pharmacy work. Methods By analyzing the rationality of prescriptions for outpatients with chronic diseases, clinical pharmacists participated in medical insurance management,reviewed the rationality of treatment programs of outpatient chronic disease and controlled medical insurance costs. Results The innovated mode of clinical pharmacist in reviewing therapeutic regimen for outpatients with chronic diseases not only cut back expenses on medicine, controlled the increase of medical insurance expenses and improved the rationality of outpatients＇ prescriptions, but also provided a new mode for clinical pharmacists to take part in the team management of chronic diseases. Conclusion Chronic disease management will be an important aspect and a platform in pharmacists future work. It supplements internal medicine services, assists medical teams in their manage- ment of chronic diseases, and at the same time it plays an increasingly important role in guaranteeing medication safety and controlling the increase of patients＇ medical expenses.
