摘要
目的探讨中药周期疗法对子宫内膜异位症腹腔镜手术后患者疗效及免疫功能的影响。方法腹腔镜手术后的子宫内膜异位症患者108例随机分为两组,观察组54例,对照组54例。两组患者均于术后第3天开始服用米非司酮片12.5毫克/次,1次/天。观察组在上述治疗的基础上加用中药周期疗法,根据月经期、排卵期、平时服用不同的中药。两组共治疗6月。评价疗效及不良反应。治疗前后测定患者CD_4^+T和CD_8^+T。结果对照组有2例患者因疗效不满意而退出本研究,观察组有1例患者因无法坚持长期中药治疗而退出,剔除者<10%,重新分析资料两组无显著性(P>0.05)。治疗6个月后观察组总有效率94.3%;对照组总有效率82.7%;观察组总有效率明显高于对照组(χ2=6.52,P<0.05)。观察组有4例发生不良反应,发生率7.5%;对照组有11例发生不良反应,发生率21.2%;观察组不良反应发生率明显小于对照组(χ~2=5.78,P<0.05)。观察组治疗前CD_4^+、CD_8^+、CD_4^+/CD_8^+分别为(20.7±2.6)%、(23.9±2.1)%、(0.81±0.12);治疗后分别为(37.5±3.5)%、(24.1±2.2)%、(1.51±0.17)。对照组治疗前CD_4^+、CD_8^+、CD_4^+/CD_8^+分别为(21.2±2.4)%、(24.2±2.2)%、(0.82±0.15);治疗后分别为(31.1±3.1)%、(24.3±2.4)%、(1.23±0.11)。治疗前两组全血CD_4^+、CD_8^+、CD_4^+/CD_8^+无明显差异(P>0.05);治疗后两组全血CD_4^+均有明显升高(t=3.172、3.902,P<0.05),两组CD_8^+无明显差异(t=1.014、1.114,P>0.05);治疗后两组CD_4^+/CD_8^+均有明显升高(t=3.776、5.128,P<0.05),并且观察组明显高于对照组(t=2.774,P<0.05)。结论中药周期疗法可以提高子宫内膜异位症患者疗效,减少不良反应,改善免疫功能。
Objective To study the effect of Chinese medicine cycle therapy on the curative effect and immune function of endometriosis after laparoscopic surgery.Methods 108 cases of endometriosis after laparoscopic surgery were randomly divided into two groups,54 cases in the observation group and 54 cases in the control group.Patients in the two groups were treated with Mifepristone Tablets 12.5 mg/times,1 times/d after surgery.The observation group on the basis of the above treatment,plus traditional Chinese medicine cycle therapy,according to menstrual period,ovulation period,usually taking different Chinese Medicine.Two groups were treated for six months.Efficacy and adverse reactions were evaluated.CD_4＋T and CD_8＋T were measured before and after treatment.Results In the control group,2 patients withdrew from the study because of unsatisfactory treatment.1 patients in the observation group were unable to adhere to long-term Chinese medicine treatment,and 10% of the patients were excluded.The data were re analyzed,and there was no significant difference between the two groups（P〈0.05）.After 6 months treatment,the total effective rate was 94.3% in the observation group and 82.7% in the control group.The total effective rate of the observation group was significantly higher than that of the control group（χ2=6.52,P〈0.05）.There were 4 cases of adverse reactions in the observation group,the incidence rate was 7.5%.There were 11 cases of adverse reactions in the control group,the incidence rate was 21.2%.The incidence of adverse reactions in the observation group was significantly smaller than that in the control group（χ2=5.78,P〈0.05）.The observation group before treatment,CD_4＋,CD_8＋,CD_4＋/CD_8＋,respectively（20.7±2.6）%,（23.9±2.1）%,（0.81±0.12） and after treatment,respectively（37.5±3.5）%,（24.1±2.2）%,（1.51±0.17）.The control group before treatment,CD_4＋,CD_8＋,CD_4＋/CD_8＋,respectively（21.2±2.4）%,（24.2±2.2）%,（0.82±0.15） and after treatment,respect
