摘要
目的:探讨血栓弹力图(TEG)在产后出血产妇输血治疗中的应用价值。方法:选取产后出血的81例产妇,采用随机数字表法分为观察组和对照组,其中观察组37例,对照组44例。两组均采用检测TEG和常规凝血,观察组根据TEG检测结果予以输血,对照组根据常规凝血检测结果予以输血,比较两组检测输血阳性率,血浆、PLT和冷沉淀输入量,ICU住院时间和总住院时间。结果:观察组TEG检测输血阳性率高于对照组;观察组患者的血浆、PLT和冷沉淀输入量均少于对照组(P<0.05),两组红细胞输入量比较差异无统计学意义(P>0.05);观察组产妇的ICU住院时间(3.3±1.3)d小于对照组的(5.9±2.0)d(P<0.05),而两者的总住院时间差异无统计学意义(P>0.05)。结论:与常规凝血检测比较,产后出血产妇采用TEG检测具有较好的预测性,并能减少输血量及入住ICU时间等优点。
Objective To explore application values of thrombelastogram ( TEG) in postoperative transfusion therapy in the department of obstetrics. Methods: 80 patients with postoperative transfusion therapy were selected and divided into observation group (n = 37) and control group ( n = 44) according to the random number table method. Both groups were given TEG and routine coagula-tion test. Besides, the observation group was given the transfusion therapy based on the routine coagulation test results. The transfusion positive rates, the plasma, PLT and cryoprecipitate contents, ICU time and total hospitalization time of the two groups were compared. Results: The transfusion positive rate of the observation group was higher than that of the control group, and the plasma, PLT and cryoprecipitate contents were less than those of the control group (P〈0.05); however, there was no statistical difference in the RBS a-mount between the two groups ( P〉0. 05). Further, the ICU time of the observation group (3.3±1. 3 d) was less than that of the con-trol group (5. 9±2. 0 d) ( P 〈0. 05 ) ; however, there was no statistical difference in the total hospitalization time between the two groups (P〉0. 05). Conclusions: Compared with the routine coagulation test, the postoperative TEG has a higher predictability, and can reduce the blood transfusion volume and shorten the ICU time.【
