摘要
目的探讨二维超声联合彩色多普勒诊断早期瘢痕子宫妊娠的临床价值。方法选取我院2014年8月至2017年8月收治的疑似瘢痕子宫妊娠患者60例平均分为两组。对照组行二维超声检查,联合组行二维超声联合彩色多普勒检查,保持追踪随访,以术后病理结果为诊断金标准,对比两组患者的诊断灵敏度、特异度、阳性及阴性预测值、符合率。结果联合组的灵敏度、阳性预测值、符合率均高于对照组(P<0.05)。治疗后,瘢痕子宫妊娠患者血HCG水平及瘢痕处包块直径均低于治疗前(P<0.05)。结论二维超声联合彩色多普勒在早期瘢痕子宫妊娠诊断中具有较高的灵敏度、阳性预测值、符合率,较单独二维超声检查更有临床优势,对患者的早期诊断和治疗具有重要意义。
Objective To explore the clinical value of two-dimensional ultrasound combined with color Doppler in the diagnosis of early scar uterus pregnancy. Methods 60 cases of patients with suspected scar uterus pregnancy in our hospital from August 2014 to August 2017 were selected and equally divided into two groups. The control group received two-dimensional ultrasound, while the combined group received two-dimensional ultrasound combined with color Doppler examination. All patients were followed up. Taking postoperative pathological results as the diagnostic gold standard, the diagnostic sensitivity, specificity, positive and negative predictive values and coincidence rate were compared between the two groups. Results The sensitivity, positive predictive value and coincidence rate of the combined group were higher than those of the control group （P〈0.05）. After treatment, blood HCG level and scar mass diameter of patients with scar uterus pregnancy were lower than those before treatment （P 〈0.05）. Conclusions Two-dimensional ultrasound combined with color Doppler has higher sensitivity, positive predictive value and coincidence rate in early diagnosis of scar uterus pregnancy, which has more clinical advantages than only two-dimensional ultrasound and is of great importance for early diagnosis and treatment of patients.
