A Comparative Study of the Two Auditory Compensation Methods on Hearing and Speech Rehabilitation in Children with Severe or Very Severe Sensorineural Hearing Loss

摘要 目的比较两种不同的听觉补偿方式对重度/极重度感音神经性语前聋患儿的听觉言语康复效果。方法收集2013年6月~2016年6月49例重度/极重度感音神经性聋患儿,初始干预均为选配助听器,其中31例作为研究组配戴助听器24 m后改行人工耳蜗植入,另18例作为对照组继续配戴助听器。以开始配戴助听器为干预起点,每隔12 m采用听觉行为分级(CAP)与言语可懂度分级(SIR)对两组患儿的听觉言语康复效果进行评估。比较干预12 m、24 m、36 m、48 m、60 m患儿的CAP、SIR得分及干预60 m各频率平均助听听阈。结果两组在配戴助听器阶段(干预12~24 m) Objective To investigate the effect of two auditory compensation methods on hearing and speech rehabilitation in children with severe or profound sensorineural hearing loss. Methods 49 cases of severe and profound sensorineural hearing loss infants were collected druing Jun. 2013-June.2016,and all of them were matched with hearing aids.31 cases of them later changed to cochlear implants after fitting poor hearing aid beyond 24m,and another 18 cases continued to wear hearing aids.To begin with start wearing a hearing aid as the starting point for intervention.the auditory and speech rehabilitation effects of two groups of hearing aid children were assessed by auditory behavioral classification (CAP) and verbal intelligibility scale (SIR) at Nikolopoulos every 12m.The CAP and SIR at intervening 12 m,24 m,36 m,48 m,60 m and the average hearing thresholds at intervening 60m between the two groups were compared. Results There were no significant differences in CAP scores between the two groups in hearing aid stage (intervention 12-24 m) (P〉0.05), and the CAP scores of the study group after cochlear implantation (intervention 36-60 m) were significantly higher than those of the control group (P〈0.05).There were no significant differences in SIR scores between the two groups in hearing aid stage (intervention 12-24 m) (P〉 0.05),and the scores of SIR in the study group after the cochlear implant (intervention 48~60 m) were significantly higher than those in the control group (P〈0.05).The mean threshold frequency intervention at 60m in the study group was significantly lower than of in the control group (P〈0.05). Conclusion Cochlear implantation is the best mode of hearing compensation for children with severe or profound sensorineural hearing loss.
