摘要
目的探讨氨甲环酸注射液在防治高血压脑出血(HCH)患者血肿再扩大中的应用价值。方法前瞻性选取2013年12月至2016年12月诊治的HCH患者100例,依据治疗方法分为A组和B组,每组50例,A组给予常规基础治疗,B组在此基础上给予氨甲环酸注射液静滴治疗。对比观察两组血肿量、美国国立卫生研究院卒中量表(NIHSS)得分、血肿再扩大率、临床疗效和不良反应。结果 A组和B组治疗后血肿量、NIHSS得分明显低于治疗前,且B组治疗后血肿量、NIHSS得分、血肿再扩大率明显低于A组,B组治疗显效率明显高于A组,差异均有显著性(P<0.05);两组不良反应发生率无显著差异(P>0.05),且均无血栓性疾病发生。结论氨甲环酸注射液治疗可有效防治HCH患者的血肿及其再扩大,有利于患者神经功能恢复,且具有良好的安全性。
Objective To explore the application of ammoniacal acid injection for prevention of the widened hematoma in patients with hypertensive cerebral hemorrhage (HCH). Methods A total of 100 patients with HCH in this hospital during December 2013 to December 2016 were selected for this prospective study,all these patients were divided into group B and group A according to the method of treatment,50 cases in each group. Patients in group A were given with regular basic treatment,and patients in group B were given with ammoniacal acid injection on the basis of regular treatment. The volume of hematoma,scores of the U. S. National Institute of Health stroke scale (NIHSS),rate of widened hema-toma,clinical efficacy and adverse reactions were observed and compared between these two groups. Results The volume of hematoma,the U. S. National Institute of Health stroke scale (NIHSS)scores after treatment in patients of group A and group B were significantly lower than those be-fore treatment,but the volume of hematoma,NIHSS scores,rate of widened hematoma in patients of group B were significantly lower than those of patients in group A,and the rate of therapeutic response in patients of group B was significantly higher than that of patients in group A (P 〈0. 05),but the rates of adverse reactions in these two groups were basically the same,and there was no thrombosis occurred (P 〉 0. 05). Con-clusion The application of injection of ammoniacal acid for treatment can effectively prevent the enlargement of hematoma in patients with hyper-ensive cerebral hemorrhage,and it is beneficial for the recovery of neurological function of patients,and it is very safe for these patients.
