摘要
目的探讨"宫颈糜烂"的分度与年龄、宫颈细胞学筛查、人乳头瘤病毒结果的关系。方法选取2013年1月-2016年8月在武汉协和医院体检中心进行过妇科体检的23 361例女性为研究对象。分别进行妇科检查及宫颈细胞学筛查(LCT)、人乳头瘤病毒检查(HPV),并分别按照宫颈"糜烂"样外观的表现、LCT结果、HPV结果及年龄分组进行研究。结果LCT结果异常占19.4%(4 192/23 361),HPV高危亚型阳性占9.3%(134/1 439)。各"糜烂"组中,LCT结果的异常及HPV高危亚型阳性的差异均有统计学意义(P<0.05);年龄的不同,宫颈外观的表现差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,年龄与"宫颈糜烂"是LCT结果正常与否的影响因素(P<0.05)。结论 "宫颈糜烂"程度的加重以及年龄的增长可增加LCT异常的风险,同时宫颈外观的异常程度随着年龄增加有减轻趋势,而HPV高危阳性率则随着其程度增加而增加。
Objective To explore the correlations between ＂ cervical erosion＂ and age,cervical cytological screening,and human papillomavirus（ HPV）. Methods A total of 23 361 women receiving gynecologial examination in Physical Examination Center,Union Hospital Affiliated to Tongji Medical College,Huazhong University of Science and Technology from January 2013 to August 2016 were selected,gynecologial examination,liquid-based cytology test（ LCT）,and HPV test were carried out,respectively,then the women were divided into different groups according to the degree of ＂ cervical erosion＂,LCT results,HPV test results,and age. Results Among the women,4 192 women（ 19. 4%,4 192/23 361） were found with abnormal results of LCT,and 134 women（ 9. 3%,134/1 439） were found with positive high-risk HPV subtypes. In ＂ cervical erosion＂ group,there were statistically significant differences in abnormal LCT results and positive high-risk HPV subtypes（ P0. 05）. There was statistically significant difference in the degree of ＂ cervical erosion＂ among different age groups（ P0. 05）. Multivariate logistic regreesion analysis showed that age and ＂cervical erosion＂ were the influencing factors of abnormol LCT results（ P0. 05）. Conclusion The aggravation of degree of ＂ cervical erosion＂ and increase of age can increase the risk of abnormal LCT results,the degree of ＂ cervical erosion＂ decreases with age and increase with increase of positive rate of high-risk HPV subtypes.
