摘要
目的探讨早期康复活动对重症监护病房(ICU)呼吸衰竭患者康复效果的影响。方法选取2015年1月至2017年5月我院ICU病房收治的呼吸衰竭患者128例,按照随机数字表法将其分为对照组和早期康复组,每组各64例,对照组行常规康复活动,早期康复组在对照组基础上进行早期康复活动,观察两组干预前后临床症状改善情况,测定两组肺功能,包括第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、计算FEV1/FVC比值,同时比较两组机械通气时间、ICU治疗时间、总住院时间及相关并发症发生情况。结果早期康复组干预后心率(HR)、呼吸频率(RR)、二氧化碳分压(PCO2)水平均低于对照组,氧分压(PO2)水平高于对照组,差异有统计学意义(P<0.05);早期康复组干预后FEV1、FVC、FEV1/FVC水平均高于对照组,差异有统计学意义(P<0.05);早期康复组机械通气时间、ICU治疗时间、总住院时间均短于对照组,差异有统计学意义(P<0.05);早期康复组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论早期康复活动的开展,有利于改善呼吸衰竭患者肺功能,促进患者康复,且能降低并发症发生率。
Objective To explore the effect of early rehabilitation activities on patients with respiratory failure in intensive care unit( ICU). Methods 128 patients with respiratory failure who were treated in ICU wards of our hospital from January 2015 to May2017 were selected and divided into control group and early rehabilitation group by random number table method,with 64 patients in each group. Patients in the control group were treated with routine rehabilitation activities,while those in the early rehabilitation group were treated with early rehabilitation activities on the basis of the control group. The improvement of clinical symptoms before and after intervention was observed in the two groups,and the lung function of both groups was measured,including forced expiratory volume( FEV1),forced vital capacity( FVC),and the ratio of FEV1 to FVC was calculated. Mechanical ventilation time,ICU treatment time,total hospitalization time and related complications were compared between the two groups. Results Levels of heart rate( HR),respiratory rate( RR) and partial pressure of carbon dioxide( PCO2) in the early rehabilitation group after intervention were lower than those of the control group,and level of oxygen partial pressure( PO2) was higher than that of the control group,with statistically significant differences( P〈0. 05). Levels of FEV1,FVC and FEV1/FVC in the early rehabilitation group after intervention were higher than those in the control group,with statistically significant differences( P〈0. 05). Mechanical ventilation time,ICU treatment time and total hospitalization time of the early rehabilitation group were shorter than those of the control group,and the differences were statistically significant( P〈0. 05). Incidence of complications in the early rehabilitation group was lower than that in the control group,and the difference was statistically significant( P〈0. 05).
