摘要
目的:探讨小剂量米非司酮治疗更年期功能性子宫出血的临床疗效及对血清性激素水平的影响。方法:选取2015年4月~2016年10月本院妇产科收治的更年期功能性子宫出血患者76例,根据就诊序号分为观察组和对照组,各38例。对照组给予复方醋酸棉酚片治疗,观察组给予小剂量米非司酮治疗。治疗6个月经周期后比较两组临床疗效、治疗前后子宫体积、子宫内膜厚度及血红蛋白(Hb)和性激素水平。结果:治疗后观察组总有效率为94.74%,明显高于对照组的78.95%(P<0.05);治疗后两组子宫内膜厚度均显著低于治疗前,观察组明显低于对照组(P<0.05或P<0.01),两组子宫体积治疗前后均无明显变化(P>0.05);治疗后两组FSH、LH及E2水平均明显降低,Hb水平均明显升高(P<0.05或P<0.01),且两组间差异显著(P<0.05或P<0.01)。结论:米非司酮治疗更年期功能性子宫出血可有效调节体内性激素水平,抑制子宫内膜生长,临床疗效显著。
Objective： To study the clinical effect of mifepristone on 38 cases of climacteric dysfunctional uterine bleeding, and its influence on serum levels of sex hormones. Methods： 76 patients with climacteric dysfunctional uterine bleeding from April 2015 to October 2016 in our hospital were divided into observation group and control group according to the access number. The control group was treated with compound gossypol acetate tablets, and the observation group was given low-dose mifepristone. The clinical effect, uterine volume, endometrial thickness, hemoglobin （Hb） and sex hormone levels were compared before and after treatment for 6 menstrual cycles. Results ： The total effective rate of the observation group was 94.74%, which was significantly higher than 75.95% of the control group （P〈0.05）. After treatment, the endometrial thickness in the 2 groups was significantly lower than before treatment, and the observation group was significantly lower than the control group （P〈0.05 or P〈0.01 ）, there was no significant change on uterine volume in the 2 groups before and after treatment （P〉0.05）. After treatment, the levels of FSH, LH and E2 were significantly decreased, the level of Hb was significantly increased （P〈0.05 or P〈0.01 ）, and the difference between, the 2 groupswas significant （P〈 0.05 or P〈0.01 ）. Conclusion： Mifepristone treatment of climacteric dysfunctional uterine bleeding can effectively regulate the level of sex hormones in patients, inhibition of endometrial growth, clinical efficacy is significant.
