摘要
目的研究布地奈德粉吸入剂对咳嗽变异性哮喘气道高反应性、炎症因子和免疫功能的影响。方法选取2015年9月至2016年9月于上海市浦东新区公利医院接受治疗的咳嗽变异性哮喘患者67例作为治疗组,67例同期健康体检者作为健康组。治疗组给予布地奈德粉吸入剂吸入治疗,评估治疗组的PD20-FEV1值,检测两组观察者不同时间段诱导痰白细胞介素(IL-5)、IL-10水平及嗜酸性粒细胞百分比(EOS%),血清炎症因子指标、免疫功能指标,并进行统计学分析。结果 (1)治疗组的PD20-FEV1值随着治疗时间延长逐步升高,3个月后水平明显高于治疗前,差异有统计学意义(P<0.05)。(2)健康组的诱导痰IL-5、IL-10水平及EOS%明显优于治疗组治疗前,差异有统计学意义(P<0.05);治疗组在治疗6、9、12个月后诱导痰IL-5、IL-10水平及EOS%与治疗前相比差异有统计学意义(P<0.05)。(3)两组治疗前的IL-6及TNF-α水平差异有统计学意义(P<0.05);经治疗12个月后,治疗组IL-6及TNF-α水平较治疗前明显改善,差异有统计学意义(P<0.05)。(4)健康组的CD3^+T细胞百分比、CD4^+T细胞百分比、CD8^+T细胞百分比及CD4^+/CD8^+比值均明显优于治疗组治疗前,差异具有统计学意义(P<0.05);经治疗12个月后,治疗组CD3^+T细胞百分比、CD4^+T细胞百分比、CD8^+T细胞百分比及CD4^+/CD8^+比值明显高于治疗前,差异有统计学意义(P<0.05)。结论布地奈德粉吸入剂治疗咳嗽变异性哮喘有助于改善患者肺功能,减轻气道高反应性,降低炎症因子水平,提高免疫功能,临床上值得推广。
Objective To research the influence of Budesonide Powder for Inhalation on airway hyperresponsiveness,inflammatory factors and immune function in the Treatment of Cough Variant Asthma.Methods 67 cases with cough variant asthma in Pudong New Area Gongli Hospital from September 2015 to September 2016 were selected as treatment group,67 healthy persons were selected as healthy group.The treatment group was given Budesonide Powder for Inhalation,the were airway hyperresponsiveness,inflammatory factors and immune function compared.Results(1)The PD20-FEV1 of the treatment group increased gradually after3 months,with statistical significance(P<0.05);(2)Before treatment,the IL-5,IL-10 content and the percentage of EOS in induced sputum of treatment group was significantly different from the healthy group,with statistical significance(P<0.05);after 6,9,12 months,the IL-5,IL-10 content and EOS was significantly different than before,with statistical significance(P<0.05);(3)Before treatment,the IL-6 and TNF-αof treatment group was significantly different from the healthy group,with statistical significance(P<0.05);after 12 months,the IL-6 and TNF-αwas significantly different than before,with statistical significance(P<0.05);(4)Before treatment,the CD3^+,CD4^+,CD8^+T cells and CD4^+/CD8^+of treatment group was significantly different from the healthy group,with statistical significance(P<0.05);after 12 months,the CD3^+,CD4^+,CD8^+T cells and CD4^+/CD8^+was significantly different than before,with statistical significance(P<0.05).Conclusion Budesonide Powder for Inhalation had certain clinical effect in treating bronchial asthma,can effectively reduce the serum levels of inflammatory cytokines,improve the pulmonary function and immunological function.
