摘要
目的:探讨分析急性脑梗死患者的大脑中动脉微栓子数量与血管性认识障碍的相关性研究。方法:选取我院收治的60例急性脑梗死患者作为病例组,选取同期我院的30例体检健康者作为对照组,通过接受TCD检查结果将病例组分为极度狭窄组、重度狭窄组和中度狭窄组,监测并记录大脑中动脉起始段的微栓子数量,采用蒙特利尔认知评估量表(MoCA)评估受试者的认知功能损害程度,分析大脑中动脉微栓子数量与狭窄程度、MoCA评分之间的相关性。结果:对照组、极度狭窄组、重度狭窄组和中度狭窄组患者的微栓子检出率、认知功能障碍情况差异皆具有统计学意义(P<0.05),大脑中动脉微栓子数量与狭窄程度呈正相关关系(r=0.868,P<0.05),与MoCA评分呈负相关关系(r=-0.859,P<0.05)。结论:ACI患者大脑中动脉微栓子数量与狭窄程度呈正相关,与MoCA评分呈负相关,大脑中动脉微栓子数量越多,认知功能障碍的程度越严重。
Objective: To investigate the correlation between the number of cerebral arterial microembolic and vascular cognitive impairment in patients with acute cerebral infarction( ACI). Methods: 60 cases of acute cerebral infarction patients in our hospital were selected as the case group,while 30 healthy cases were selected as the control group. Through the examination results of TCD,ACI patients in case group were divided into extreme stenosis group,severe stenosis group and moderate stenosis group. The number of microemboli in the initial segment of middle cerebral artery was monitored and recorded. The degree of cognitive impairment was assessed with Montreal cognitive assessment scale( Mo CA). The correlation between the number of cerebral arterial microemboli,the degree of stenosis and the Mo CA were analyzed.Results: The detection rate of microemboli and cognitive impairment in the control group,extreme stenosis group,severe stenosis group and moderate stenosis group were statistically different( P〈0. 05). The number of cerebral arterial microemboli and the degree of stenosis were positively correlated( r = 0. 868,P〈0.05),but it was negatively correlated with Mo CA( r =-0.859,P〈0.05). Conclusion: The number of cerebral arterial microemboli and the degree of stenosis in ACI patients are positively correlated,and negatively correlated with Mo CA. The greater the number of cerebral arterial microemboli,the more serious degree of cognitive impairment for ACIpatients.
