低钙透析液对维持性血液透析患者血清钙磷代谢的影响

摘要 目的探讨使用低钙透析液治疗对维持性血液透析患者钙磷代谢及血清甲状旁腺激素(iPTH)的影响。方法选择血清iPTH<150 pg/mL的维持性血液透析患者20例,使用钙离子浓度为1.25 mmol/L的低钙透析液治疗。观察治疗前及治疗后3、6个月时透析前后血清矫正钙、磷及iPTH水平变化。结果低钙透析液治疗3个月后患者透析前后血钙水平较前下降,iPTH水平明显升高,治疗6个月后水平进一步升高,与治疗前相比均有统计学意义。血磷水平无明显变化。皮肤瘙痒患者症状明显减轻。结论对于血清iPTH<150 pg/mL或高钙血症及皮肤瘙痒的患者,使用低钙透析液可减轻患者体内钙负荷,改善甲状旁腺功能,为含钙磷结合剂的使用提供更大空间,且不良反应较少。 Objective To study the effects of low calcium dialysate on calcium and phosphorus metabolism and iPTH in maintenance hemodialysis patients. Methods Twenty patients with maintenance hemodialysis with serum par- athyroid hormone 〈150 pg/mL were treated with low calcium dialysate with a calcium ionconcentration of 1.25 mmol/L. The changes of serum calcium, phosphorus and parathyroid hormone levels were observed before and after dialysis before and 3 and 6 months after treatment. Results After 3 months of treatment with low calcium dialysate, the levels of serum calcium were decreased before and after dialysis, the level of parathyroid hormone was significantly increased, and the level increased slightly after 6 months of treatment, which was statistically significant compared with before treatment. There was no significant change in serum phosphorus levels. Skin itching patients with symptoms significantly reduced. Conclusion The use of low calcium dialysate in patients with serum parathyroid hormone 〈150 pg/mL or hypercalcemia and skin itching can reduce the calcium burden in patients and improve parathyroid function, providing more calcium and phosphorus binders Large space, and less side effects.
