摘要
目的探讨尿毒症长期透析患者伴异位钙化的影像学表现,进一步提高对该病的认识。方法回顾分析我院20例尿毒症长期透析患者(腹透者16例,血透者3例,腹透后转血透者1例)临床、病理及影像学资料,分析不同部位异位钙化的影像学表现。结果 20例患者均为多部位异位钙化。主动脉(20例)、冠状动脉(9例)、肠壁血管(1例)、表现为不同程度的斑点状、环形或半环形钙化,部分呈"动脉造影征"阳性,心瓣膜呈斑片状钙化5例。肺内呈网格状钙化2例。大关节周围结节、肿块样钙化12例,经病理证实均为肿瘤样钙质沉积,影像表现为软组织内肿块呈"卵石样""流注状"及"沉降征"等。结论长期透析治疗的尿毒症患者易出现异位钙化灶,不同部位的异位钙化、肿瘤样钙质沉积具有相对特征性影像学表现,结合病史不难做出诊断。
Objective To investigate the imaging manifestations of uremic patients with Long-term dialysis treatmen accompanied by ectopic calcification so as to further improve the diagnostic level for this disoder. Methods The clinical,pathological and radiological data of 20 uremia patients with long-term dialysis from our hospital,of whom 16 were subjected to peritoneal dialysis and 3 to,1 first to peritoneal dialysis and then to hematodialysis,were retrospectively analyzed. The imaging findings of ectopic calcification in different parts were also analyzed in this work. Results All those 20 patients exhibited multiple ectopic calcification. The aorta calcification was found in 20 cases,coronary artery calcification in 9 cases,and diffuse intestinal vascular calcification in 1 case,which displayed varying degrees of spot-like,ring or semicircle shape of calcification,and certain presented with arteriography sign. 5 cases displayed spot-like shape of calcification in heart valve and 2 cases displayed latticed shape of calcification in lung. The nodules or,tumour-like calcification around the large joints were found in 12 cases,which were pathologically confirmed tumor-like calcium deposition. The imagings manifested ＂ pebble-like＂, ＂ streamer-like＂ and ＂ settlement＂ sign,and so on,inside the soft tissue masses. Conclusion The uremic patients with long-term dialysis are prone to ectopic calcification. The ectopic calcification in different parts and tumor-like calcium deposits possess relatively characteristic imaging features. In combination with history,the disorder is not difficult to make a diagnosis.
