右美托咪定对骨科手术患者椎管内麻醉后寒战反应的预防作用观察 被引量:4

摘要 目的观察右美托咪定对骨科手术患者椎管内麻醉后寒战反应的预防作用。方法选择在我院择期进行骨科手术患者78例,且均行椎管内麻醉。按随机数表法分为两组,各39例。对照组采用静脉注射生理盐水,观察组选用右美托咪定实施麻醉,比较两组寒战反应分级及镇静评分标准(Ramsay)评分。结果观察组寒战反应分级0级36例(92.3%)、1级(7.7%)、2级、3级、4级均为0例(0.0%),与对照组25(64.1%)、7例(17.9%)、5例(12.8%)、2例(5.1%)、0例(0.0%)比较,差异有统计学意义(P<0.05);药物泵注结束(T1)-手术结束(T3)时,观察组Ramsay评分(2.9±0.7)分、(2.8±0.7)分、(2.5±0.6)分较对照组(2.2±0.4)分、(2.2±0.5)分、(2.0±0.4)分明显升高,差异有统计学意义(P<0.05)。结论给予骨科手术患者右美托咪定实施椎管内麻醉可有效预防寒战反应发生,提高镇静效果。 derive To observe the preventive effect of dexmedetomidine on shivering after spinal anesthesia in pa-tients undergoing orthopedics surgery. Methods Seventy-eight cases of spinal anesthesia in patients undergoing orthopedics surgery in our hospital were collected and divided into control group and experimental group 〉 with each group 39 cases. The patients in the control group received intravenous saline treatment, and the patients in the experimental group received dexme-detomidine treatment. The shivering reaction scale and sedation score standard (Ramsay) score were compared between the two groups. Results The experimental group of 36 cases of shivering 0 grade (92. 3%) , 1 grade (7. 7% ) , 2 grade, 3 grade 4 were 0 cases (0%). The control group of 25 cases of shivering 0 grade (92. 3%),7 cases of 1 grade (17. 9%),5 cases of 2 grade (12. 8%) , 2 cases of 3 grade (5. 1%) and 0 cases of 4 grade (0%) was statistically significant compared with experi-mental group (P〈0 . 05). Drug infusion end (Tl ) - the end of operation (T3) ? Ramsay score of the experimental group (2. 9 ±0. 7),( 2. 8±0. 7),(2. 5d=0. 6) % was significantly increased compared with the control group (2. 2±0. 4) , (2. 2±0. 5 ) ,(2±0. 4). The difference was statistically significant (P〈0. 05). Conclusion The effect of dexmedetomidine on spinal anes-thesia in patients undergoing of orthopedics surgery could effectively prevent shivering and improve the sedative effect.
