摘要
目的分析Toll样受体2,4(TLR2,TLR4)在过敏性紫癜(HSP)患儿外周血单个核细胞(PBMC)细胞中的表达,并探讨TLR2和TLR4水平与患儿T细胞亚群的关系。方法选取2015年3月~2017年2月上海儿童医学中心收治的经确诊为HSP的患儿65例为研究对象,其中单纯HSP 49例(HSP组),伴肾脏受累的16例(HSPN组);选取同期体检健康的儿童30例为对照组。采用实时荧光定量PCR检测PBMC细胞中TLR2和TLR4的表达;采用流式细胞仪检测Th1,Th2,Th17和Treg细胞比例,以及Th1/Th2,Th17/Treg比值;采用Pearson法分析TLR2,TLR4表达与Th1/Th2,Th17/Treg比值的相关性。结果 HSPN组,HSP组和对照组TLR2表达分别为2.13±0.36,1.64±0.27和1.05±0.12,差异有统计学意义(F=129.630,P<0.01);TLR4表达分别为2.21±0.45,1.82±0.34和1.13±0.17,差异有统计学意义(F=81.241,P<0.01)。HSPN组Th1,Treg细胞比例以及Th1/Th2比值均明显低于HSP组和对照组,差异有统计学意义(F=34.103,63.828,91.432,均P<0.01);HSPN组Th2,Th17细胞比例以及Th17/Treg比值均明显高于HSP组和对照组,差异有统计学意义(F=48.559,48.475,34.909,均P<0.01)。TLR2,TLR4与Th1/Th2比值呈负相关(r=-0.601,-0.652,P<0.01),与Th17/Treg比值呈正相关(r=0.617,0.712,P<0.01)。结论 HSP患儿PBMC细胞TLR2,TLR4表达增加,且可能与患儿Th1/Th2,Th17/Treg免疫应答失衡有关。
Objective To analyze the expression of toll like receptor 2/4（TLR2,TLR4）in peripheral blood mononuclear cells （PBMC）cells of children with henoch schonlein purpura（HSP）,and investigate its relationship with T-cell subgroups. Methods A total of 85 HSP children treated in Shanghai Children＆#39;s Medical Center from March 2015 to February 2017 were selected,including 49 children without renal failure（HSP group）and 16 children with renal failure（HSPN group）.30 healthy children were enrolled as control group.The expression of TLR2,TLR4 was examined by real-time PCR.The ratio of Th1,Th2,Th17,Treg,Th1/Th2 and Th17/Treg was detected by flow cytometry.The relationship between TLR2,TLR4 and Th1/Th2,Th17/Treg was analyzed.Results The expression of TLR2 in HSPN,HSP and control group were 2.13± 0.36,1.64±0.27 and 1.05±0.12,the expression of TLR4 were 2.21±0.45,1.82±0.34 and 1.13±0.17 respectively.The differences were statistically significant（F=129.630,81.241,all P〈0.01）.The percentage of Th1,Treg cells and the ratio of Th1/Th2 in HSPN group were significantly lower than those in HSP group and control group,and the differences were statistically significant（F=34.103,63.828,91.432,all P〈0.01）.The percentage of Th2,Th17 cells and the ratio of Th17/Treg in HSPN group were significantly lower than those in HSP group and control group,and the differences were statisti-cally significant（F=48.559,48.475,34.909,P〈0.01）.The expression of TLR2 and TLR4 were negatively correlated with Th1/Th2（r=-0.601,-0.652,P〈0.01）,and positively correlated with Th17/Treg（r=0.617,0.712,P〈0.01）.Conclu-sion The expression of TLR2,TLR4 increases in HSP children,and may be related to the imbalance of Th1/Th2 and Th17/Treg immune response.
