比索洛尔联合噻托溴铵对慢性阻塞性肺疾病急性加重期患者血清炎症因子及肺功能的影响 被引量:20

Effects of bisoprolol combined with tiotropium bromide on serum inflammatory factors and pulmonary function in patients with acute exacerbation of chronic obstructive pulmonary disease

摘要 目的 探讨比索洛尔联合噻托溴铵对慢性阻塞性肺疾病急性加重期 (AECOPD)患者 血清炎症因子及肺功能的影响。方法 将68例 AECOPD 患者分为观察组和对照组,每组34例。全部 患者给予常规对症治疗。对照组采用雾化吸入噻托溴铵治疗。观察组在对照组基础上,给予比索洛尔 治疗。对比2组治疗前后肺功能指标和炎症因子指标。采用 COPD 评估测试问卷评分表 (CAT)评估 患者病情变化。采用6分 钟 步 行 距 离 (6MWD) 检 测 患 者 运 动 耐 力 情 况。结 果 观 察 组 的 总 有 效 率 (91.18%)显著高于对照组 (67.65%),差异有统计学意义 (P <0.05);2组治疗后超敏 C反应蛋白 (hs-CRP)、IL-17、IL-23 的 水 平 较 治 疗 前 均 明 显 降 低 (P <0.05); 治 疗 后, 观 察 组 的 hs-CRP、IL-17、IL-23明显低于对照组,差异有统计学意义 (t=4.216、3.153、2.240,P <0.05);2组治疗 后 FEV1、呼气峰流速 (PEFR)、FEV1/FVC、6MWD 的 水 平 明 显 升 高,CAT 评 分 明 显 降 低 (P < 0.05);治疗后,观察组的 FEV1、PEFR、FEV1/FVC、6MWD 显著高于对照组,CAT 评分显著低于 对照组,差异有统计学意义 (t=4.115、4.663、8.078、5.442、3.331,P <0.05)。结论 比索洛尔 联合噻托溴铵能显著改善 AECOPD 患者的肺功能,减轻炎症反应。 Objective To investigate the effect of bisoprolol combined with tiotropium bromide on serum inflammatory factors and pulmonary function in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Methods Sixty-eight cases of AECOPD patients were divided into observation group and control group, each group of 34 cases, All patients were given conventional symptomatic treatment. The control group was treated with tiotropium bromide inhalation. The observation group in the control group on the basis of bisoprolol treatment. The pulmonary function indexes and inflammatory factors were compared between the two groups before and after treatment. The patients were assessed by the chronic obstructive pulmonary disease (COPD) evaluation questionnaire (CAT). 6 minutes walking distance (6MWD) was used to measure the exercise endurance. Results The total effective rate of the observation group (91.18%) was significantly higher than that of the control group (67.65%). The difference was statistically significant ( P 〈0.05). After treatment, the levels of hs-CRP, IL-17 and IL-23 in the two groups were significantly lower than those before treatment ( P〈 0.05). After treatment, the hs-CRP, IL-17 and IL-23 in the observation group were significantly lower than those in the control group. The difference was statistically significant ( t =4. 216,3. 153,2. 240, P〈 0.05). After treatment, the levels of FEV1, PEFR, FEV1/FVC and 6MWD were significantly increased in two groups, and the CAT score was significantly lower ( P 〈0.05). After treatment, the FEV1 , PEFR, FEV1/FVC and 6MWD in the observation group were significantly higher than those in the control group, and the CAT score was significantly lower than that in the control group. The difference was statistically significant (t =4.115,4.663,8.078, 5.442,3.331, P〈0.05).Conelusions Bisoprolol combined with tiotropium bromide can signifieantly improve the pulmonary function of patients with AECOPD, and
