摘要
目的探讨是否规律抗癫痫治疗对癫痫患者外周血T淋巴细胞水平变化的影响及意义。方法收集2016年9月至2017年8月黔江民族医院住院及门诊就诊的癫痫患者300例,根据是否服用抗癫痫药物分为未服药组和规律服药组,各150例,同时选取同期门诊健康体检者或志愿者150例为对照组。应用流式细胞仪检测3组患者外周血中CD3^+、CD4^+、CD8^+T淋巴细胞水平,计算CD4^+/CD8^+比值;采用免疫透射比浊法测定3组患者血浆免疫球蛋白IgG、IgA、IgM水平。结果与对照组比较,癫痫患者(未服药组+规律服药组)CD8^+T淋巴细胞比例升高,CD3^+、CD4^+T淋巴细胞比例及CD4^+/CD8^+比值降低(P<0.05,P<0.01);与未服药组比较,规律服药组CD3^+、CD4^+T淋巴细胞比例及CD4^+/CD8^+升高,CD8^+T淋巴细胞比例降低(P<0.05,P<0.01)。与对照组比较,癫痫患者(未服药组+规律服药组)血浆IgG、IgA等免疫球蛋白含量降低(P<0.05,P<0.01);与未服药组比较,规律服药组IgA含量升高(P<0.01)。结论癫痫患者体内普遍存在细胞和体液免疫功能紊乱,规律服用抗癫痫药物可改善免疫功能失衡现象。
Objective To investigate the effect and significance of regular anti-epileptic therapy on peripheral blood T lymphocyte levels in epileptic patients. Methods A total of 300 cases of epileptic patients who received treatment in Chongqing Qianjiang National Hospital from September 2016 to August 2017 were divided into non-medication group and regular medication group according to whether they took anti-epilepsy drugs (150 cases each) , and 150 cases of healthy examinees or volunteers at the same time were selected as control group. Flow cytometer was used to detect the level of CD3^+ , CD4^+ and CD8^+ T lymphocytes in peripheral blood of 3 groups, and the ratio of CD^4+ and CD8^+ was calculated. Turbidimetric inhibition immuno assay was used to detect the plasma immunoglobulin IgG, IgA and IgM level in of 3 groups' patients. Results Compared to control group, the levels of CD8^+ were higher in epileptic patients ( non-medication group and regular medication group), and the level of CD3^+ , CD4^+ and the ratio of CD4^+ and CD8^+ were lower in epileptic patients ( P 〈 0.05, P 〈 0.01 ). Compared to the non-medication group, the level of CD3^+ , CD4^+ and the ratio of CD4^+ and CD8^+ were higher in regular medication group, and the level of CD8^+ in non-medication group was lower than that in regular medication group ( P 〈 0.05, P 〈0.01 ). The level of plasma IgA and IgG in epileptic patients were lower than those in control group ( P 〈 0.05 ,P 〈 0.01 ). The level of IgA in regular medication group was higher than that in non- medication group ( P 〈 0.01 ). Conclusion Cellular and humoral immune dysfunction are generally exist in epileptic patients, and the regular use of anti-epileptic drugs can improve the imbalance of immune function.
