摘要
目的分析不同草绿色链球菌的分布及对常用抗菌药物的耐药情况。方法收集无菌部位临床标本分离的菌株,采用Phoenix ^(TM)-100全自动细菌检测分析系统进行细菌鉴定和药敏试验。结果共收集213株常见草绿色链球菌,包括咽峡炎链球菌89株(41.8%),星座链球菌45株(21.1%),缓症链球菌45株(21.1%),唾液链球菌34株(16.0%)。草绿色链球菌主要分离自腹水、血、胸水、中段尿和脑脊液,分别为100株(46.9%)、56株(26.3%)、29株(13.6%)、24株(11.3%)和4株(1.9%)。草绿色链球菌对青霉素和阿莫西林的敏感率分别为90.8%和90.5%,对红霉素、克林霉素、四环素的耐药率分别为72.9%、63.4%和61.5%。咽峡炎链球菌和缓症链球菌对左氧氟沙星的敏感率分别为82.0%和72.7%。所有菌株对头孢噻肟、头孢吡肟及美罗培南敏感率高,未见对万古霉素和利奈唑胺耐药菌株。结论建议临床治疗草绿色链球菌引起的感染时,优先选择青霉素、头孢噻肟或头孢曲松等,大环内酯类、克林霉素和四环素的耐药率比较高,不建议使用。氟喹诺酮类药物对草绿色链球菌的敏感性较青霉素和头孢噻肟低,不宜用于一线抗感染治疗。
Objective Analysis of the distribution of different Viridans streptococcus and antibiotic resistance to commonly used antimicrobial agents.Methods The Viridans streptococcus strains were isolated from sterile site clinical specimens,the identification and antimicrobial susceptibility test were completed on PhoenixTM-l00.Results A total of 213 strains of Viridans streptococcus were collected,including S.anginosus 89(41.8%),S.constellatus 45(21.1%),S.mitis 45(21.1%) and S.salivarius 34(16.0%).The isolates were mainly from ascites (46.9%),blood (26.3%),hydrothorax (13.6%),urine (11.3%),and cerebrospinal fluid (1.9%).90.8% of Viridans streptococcus were susceptible to penicillin and 90.5% to amoxicillin.The percentage of resistance to erythromycin,clindamycin and tetracycline was 72.9%,63.4% and 61.5%.82.0% of S.anginosus and 72.7% of S.mitis were susceptible to levofloxacin.All strains were susceptible to cefotaxime,cefepime and meropenem.No strains were resistant to vancomycin and linezolid.Conclusion It was suggested that the clinical treatment of infections caused by Viridans streptococcus,should prefer penicillin,cefotaxime,or ceftriaxone.Macrolides,clindamycin and tetracycline resistance rates were higher,and thus were not recommended.The sensitivity of Viridans streptococcus to fluoroquinolones was lower than to that to the penicillin and cefotaxime,and therefore fluoroquinolones should not be the first choice of the treatment.
