摘要
目的:观察重复经颅磁刺激(r TMS)对原发性失眠的疗效。方法:80例原发性失眠患者随机分为研究组(43例,完成41例)和对照组(37例,完成34例),研究组给予右侧额叶背外侧区低频(1 Hz)r TMS治疗;对照组予假刺激(0 Hz,部位同研究组)治疗;疗程10 d,未予药物治疗。治疗前后采用匹兹堡睡眠质量指数(PSQI)和焦虑自评量表(SAS)、抑郁自评量表(SDS)评估,治疗后采用不良反应量表(TESS)评估。结果:研究组治疗有效率显著高于对照组(χ~2=7.440,P<0.05),不良反应发生率显著低于对照组(χ~2=5.932,P<0.05)。协方差分析结果示,研究组治疗后PSQI睡眠质量因子、入睡时间因子、睡眠时间因子、日间功能障碍因子和总分的改善、以及SAS量表减分较对照组更为显著(P均<0.05)。结论:r TMS治疗原发性失眠疗效显著,对失眠伴随的焦虑症状也有较好的治疗作用。
Objective: To investigate the effect of repetitive transcranial magnetic stimulation(rTMS) on primary insomnia. Method :80 patients with primary insomnia were randomly divided into study group (43, completed 41 cases) and control group (37, completed 34 cases). The study group was treated with low frequen- cy rTMS (frequency of 1 Hz, the right side of the dorsolateral preferential cortex), while the control group was given sham stimulation( stimulation frequency of 0 Hz, the same position with the study group). The patients were treated for 10 days. Measurements including Pittsburgh sleep quality index (PSQI), self anxiety scale (SAS) and self depression scale(SDS) for clinical symptom and the treatment emergent symptom scale (TESS) for sideeffects were evaluated. Results:The effective rate in study group was significantly higher than that in control group( ~2 = 7. 440, P 〈 0.05 ). The adverse reaction rate in study group was significantly lower than that in the control group ( ~2 = 5. 932, P 〈 0.05 ). Analysis of co-variance revealed significant differences between study group and control group in the scores of sleep quality factor, sleep duration factor, the sleep latency factor, daytime dysfunction due to sleepiness factor, total score of PSQI and SAS ( all P 〈 0.05) . Conclusion: rTMS is effective in the treatment of insomnia, and has a better therapeutic effect on insomnia accompanied with anxie- ty symptoms.
