摘要
目的观察左乙拉西坦对青少年癫癇部分性发作患者精神状态及心理状态的影响。方法选取作者医院神经内科收治的72例癫癇部分性发作患者,采用随机数字表法均分为观察组(n=36)与对照组(n=36)。对照组单用奥卡西平治疗,观察组在此基础上加用左乙拉西坦。治疗16周后统计两组患者治疗期内平均每周部分性癫癇发作频率,并比较两组患者的Epworth嗜睡量表(Epworth sleepiness scale,ESS)评分、汉密尔顿焦虑量表(Hamilton anxiety scale,HAMA)评分及汉密尔顿抑郁量表(Hamilton depression scale,HAMD)评分及不良反应发生情况。结果治疗期内观察组的平均每周部分性癫癇的发作频率明显低于对照组,组间比较差异具有统计学意义(P<0.01);但是治疗结束后观察组的ESS、HAMA与HAMD评分均明显高于对照组,组间比较差异具有统计学意义(P<0.01),两组患者不良反应发生情况比较差异无统计学意义(P>0.05)。结论左乙拉西坦有助于降低青少年患者部分性癫癇发作频率,但也会引起嗜睡、焦虑及抑郁等精神和心理问题,临床使用应权衡利弊。
Objective To investigate the effects of levtiracetam on mental and psychological status of adolescent with partial epileptic seizures.Methods Seventy-two adolescents with partial epileptic seizures from department of neurology in author＇s hospital were collected as study objects,and then randomly divided into observation group（n=36）and control group（n=36）.The adolescents in the control group were treated with oxcarbazepine,and the adolescents in the observation group were used with oxcarbazepine plus levetiracetam.After 16 weeks treatment,the average weekly frequencies of partial epileptic seizures were analyzed.The scores of Epworth sleepiness scale（ESS）scores,Hamilton anxiety scale（HAMA）scores,Hamilton depression scale（HAMD）scores and adverse reactions were compared between two groups.Results The average weekly frequencies of partial epileptic seizures in the observation group were significantly lower than these in the control group（P0.01）,and ESS,HAMA and HAMD scores in the observation group were obviously higher than those in the control group（P0.01）.There was no significant difference in adverse reactions between two groups（P0.05）.Conclusion Levetiracetam helps to reduce the frequency of partial epileptic seizure in adolescents,but it also causes mental and psychological problems such as sleepiness,anxiety and depression.The clinical use of levetiracetam should weigh the pros and cons.
