摘要
目的探讨血清miR-216a水平与急性胰腺炎(AP)严重程度的相关性。方法选择广州医科大学附属乐从医院2015年6月至2016年6月收治的轻型急性胰腺炎(MAP组)17例,重型急性胰腺炎(SAP组)23例和经检查无异常的健康成人(对照组)30例,采集血液标本检测淀粉酶活性(AMYA)和血清miR-216a的表达水平,采用Ranson评分、急性生理学及慢性健康状况评分系统(APACHEⅡ)和改良的CT严重(MCTSI)指数来评价AP的严重程度,分析上述指标与血清miR-216a表达水平的相关性。结果急性期MAP组与SAP组患者血液淀粉酶活性均高于本组恢复期及对照组,差异有统计学意义(P<0.05),且急性期SAP组高于MAP组,差异有统计学意义(P<0.05);急性期MAP组和SAP组血清miR-216a均显著高于该组恢复期及对照组,差异有统计学意义(均P<0.05),且SAP组高于MAP组,差异有统计学意义(P<0.05);miRNA-216a表达水平与Ranson评分、APACHEⅡ评分和MCTSI指数呈正相关(r=0.667,P<0.05;r=0.396,P<0.05;r=0.648,P<0.05)。结论急性胰腺炎患者血清miR-216a表达水平显著高于健康者,表达水平与急性胰腺炎的严重程度呈正相关,对于SAP的诊断具有重要意义。
Objective To explore the correlation between serum miRNA-216a level and severity of acute pancretitis （AP）. Meth-ods 17 cases of mild acute pancreatitis （MAP group）,23 cases of severe acute pancreatitis （SAP group） and 30 cases of healthysubjective （control group） were selected in our hospital from June 2015 to June 2016. Blood amylase activity and serum miRNA-216a were detected,Ranson value, APACHEK value and modified CT severity index （MCTSI） were used to evaluate the severity oAP,analyze the correlation between miR-216a level and other indexes. Results The serum amylase activity of MAP group and SAPgroup in acute stage were higher than those in convalescence in these groups and the control group（P 〈0. 05） ,and the serum amyl-ase activity in the acute phase SAP group was higher than that in the MAP group, and the difference was statistically significant（P 〈0. 05）. The relative expressions of serum miR-216a in MAP group and SAP group in acute stage were significantly higher thanthose in convalescence in these groups and the control group （P 〈0. 05） , and the relative expressions of serum miR-216a in SAPgroup was higher than that in the MAP group,and the difference was statistically significant （P 〈0. 05）. The miR-216a expressionwas positively correlated with Ranson score,APACHE score and MCTSI score （r =0. 667, P〈0. 05 ; r = 0. 396, P 〈0. 05 , and r = 0. 648, P〈0. 05）. Conclusion The expression level of serum miR-216a of patients with AP was significantly higher than that ohealthy people？and the expression level of serum miR-216a was positively correlated with the severity of AP？which was useful forthe diagnosis and prognosis of SAP.
