摘要
目的评价经支气管镜削瘤联合125I粒子植入治疗气管腺样囊性癌（TACC）的临床疗效。方法回顾性分析煤炭总医院呼吸科及肿瘤科自2010年1月至2016年1月收治的行支气管镜下削瘤联合125I粒子植入治疗的TACC患者共19例，比较治疗前后的症状、气促评分、管腔直径、管壁厚度的变化，并分析并发症情况。结果所有患者经治疗后症状均改善，气促评分由术前的26±1．2降至0．2±0．5，管腔直径由术前的（5．6±1．6）mm升高至（13．4±1．1）mm，管壁厚度由术前的（29．6±1．3）mm降至（82±1．1）mm，差异均有统计学意义（P值均〈O．05）。经支气管镜植入粒子并发症为咯血、感染和粒子移位各1例；经皮穿刺植入粒子并发症为气胸2例，咯血和粒子移位各1例。结论经支气管镜削瘤联合125I粒子植入治疗TACC可以改善患者的症状，对管内＋管壁型肿瘤有较好的抑制作用。
Objective To evaluate clinical curative effect of bronchoscopy interventional therapy combined with 125I particle implantation on trachea adenoid cystic carcinoma （TACC）. Methods The data of 19 patients with TACC receiving bronchoseopy interventional therapy combined with 125I particle implantation in China Meitan General Hospital from January 2010 to January 2016 were retrospectively analyzed. The symptoms, dyspnea index, trachea diameter and thickness of the trachea wall were compared before and after treatment. The complications were also analysed. Results The symptoms of all patients were improved after treatment. The dyspnea index decreased from 2.6 ±1.2 to 0.2 ±0.5, the trachea diameter increased significantly from （5.6± 1.6） mm to （13.4±1.1） mm, and the thickness of the trachea wall decreased from （29.6±1.3） mm to （8.2± 1.1） mm, there were statistical differences in above indexes （all P〈 0.05）. The complications of particle implantation by bronchoscopy included hemoptysis, infection and particle transloeation in one case respectively. The complications of particle implantation by CT guide included pneumothorax in two cases, hemoptysis and particle translocation in one case respectively. Conclusions Treatment with bronehoscopy interventional therapy combined with 125I particle implantation on TACC can improve the symptoms of the patients and inhibit tumor growth.
