摘要
目的探讨中药熏蒸配合关节松动治疗髌骨骨折术后膝关节功能障碍的疗效,为临床治疗方案的制定提供参考。方法选取2014年6月至2016年6月72例髌骨骨折术后膝关节功能障碍患者,随机分为观察组和对照组,每组36例。观察组采用中药熏蒸配合关节松动进行治疗,对照组采用下肢关节康复器(CPM)功能锻炼。对比两组患者治疗前后患侧膝关节最大被动活动时视觉模拟评分法(VAS)的评分结果;对比两组患者治疗前后的美国特种外科医院膝关节评分(HSS)、膝关节活动度(ROM)以及日常生活活动能力(ADL)。结果两组患者治疗后的VAS评分均显著低于治疗前(P<0.05);且观察组治疗后显著低于对照组(P<0.05);两组患者治疗后HSS、ROM及ADL评分均显著高于治疗前(P<0.05),且观察组治疗后HSS、ROM及ADL评分均显著高于对照组(P<0.05)。结论中药熏蒸配合关节松动可有效减轻髌骨骨折术后膝关节疼痛、活动障碍等症状,促进膝关节功能的恢复,提高患者术后的生活质量。
Objective To investigate the effect of traditional Chinese medicine fumigation combined with joint mobilization on knee joint dysfunction after fracture of patella operation. Methods Seventy-two patients with knee joint dysfunction after patella fracture operation were selected from June 2014 to June 2016,they were randomly divided into observation group and control group,with 36 cases in each group. The observation group was treated by traditional Chinese medicine fumigation combined with joint mobilization,while the control group was treated by CPM exercise. The visual analogue scale（ VAS） scores were compared between the two groups before and after treatment with the greatest passive motion of the affected knee; the hospital for Special Surgery knee score（ HSS）,the knee joint range of motion（ ROM） and activities of daily living（ ADL） before and after treatment were compared between the two groups. Results After treatment,VAS scores of the both groups were significantly lower than those before treatment（ P〈0. 05）; and the VAS scores of the observation group after treatment were significantly lower than those of the control group（ P〈0. 05）. The HSS,ROM and ADL scores were significantly higher than those before treatment（ P〈0. 05）,and those of the observation group after treatment were significantly higher than those of the control group（ P〈0. 05）. Conclusion Traditional Chinese medicine fumigation combined with joint mobilization can effectively reduce the knee pain,movement disorders and other symptoms,and to promote the recovery of knee function and improve the quality of life of patients after operation.
