摘要
目的:探究粒细胞集落刺激因子(G-CSF)在成人噬血细胞综合征(HLH)治疗中的作用。方法:对本中心首次诊疗过程中使用G-CSF的104例成人HLH患者的临床信息进行研究。结果:使用G-CSF治疗前后患者中性粒细胞绝对值、体温、血小板绝对值、谷丙转氨酶、谷草转氨酶、总胆红素等指标差异有统计学意义,且趋于好转(P<0.05);而血红蛋白、甘油三酯、血清铁蛋白、纤维蛋白原等指标治疗前后差异无统计学意义;使用GCSF治疗期间患者未出现幼稚细胞增加、休克、间质性肺炎、成人呼吸窘迫综合征等严重的不良反应。结论:GCSF的应用不仅可以减少成人HLH治疗中粒细胞减少症的发生,并且尚未发现G-CSF的应用对成人HLH治疗中存在负面影响。
Objective：To explore the role of granulocyte colony stimulating factor（G-CSF）in the treatment of adult hemophagocytic lymphohistiocytosis（HLH）.Method：We collected the clinical information of 104 patients with HLH who used the G-CSF and conducted a retrospective study.Result：Before and after the treatment with GCSF,neutrophils,temperature,platelet absolute value,alanine aminotransferase,aspartate aminotransferase,total bilirubin and other indicators were significantly different and better.However,there were no significant differences in indicators,such as hemoglobin,triglyceride level,serum ferritin level and fibrinogen.There were no severe adverse reactions such as increase of immature cells,shock,interstitial pneumonia,adult respiratory distress syndrome and so on during G-CSF treatment.Conclusion：The application of G-CSF can not only reduce the incidence of neutropenia in adult HLH therapy,but it also has not yet been found that the application of G-CSF has a negative impact on the treatment of adult HLH.
