摘要
目的分析在脑卒中后偏瘫肩痛患者中使用经皮神经电刺激(TENS)结合肩胛带训练进行治疗的效果。方法 70例脑卒中后偏瘫肩痛患者,随机分为对照组与观察组,每组35例。对照组采用经皮神经电刺激疗法,观察组在此基础上加用肩胛带训练。对比两组患者的治疗效果。结果治疗前,两组简式Fugl-Meyer运动功能评分(FMA评分)比较差异无统计学意义(P>0.05);治疗后,观察组患者的FMA评分(45.10±3.09)分明显高于对照组的(28.63±2.90)分,差异具有统计学意义(P<0.05)。治疗前,两组改良Barthel指数(MBI)评分比较差异无统计学意义(P>0.05);治疗后,观察组患者的MBI评分(79.63±9.31)分明显高于对照组的(52.29±9.42)分,差异具有统计学意义(P<0.05)。治疗前,两组视觉模拟评分法(VAS)评分比较差异无统计学意义(P>0.05);治疗后,观察组患者的VAS评分(2.53±0.52)分明显低于对照组的(5.48±0.68)分,差异具有统计学意义(P<0.05)。结论对脑卒中后偏瘫肩痛患者在经皮神经电刺激疗法基础上加用肩胛带训练疗法,有助于进一步降低患者疼痛水平,对于提高其生活质量具有积极意义,疗效较佳,可推广。
Objective To analzye the efficacy of transcutaneous electrical nerve stimulation(TENS) combined with shoulder girdle training in the treatment of patients with hemiplegic shoulder pain after stroke. Methods A total of 70 patients with hemiplegic shoulder pain after stroke were randomly divided into control group and observation group, with 35 cases in each group. The control group received transcutaneous electrical nerve stimulation, and the observation also received shoulder girdle training. The treatment effect in two groups was compared. Results Before treatment, both groups had no statistically significant difference in simplified Fugl-Meyer motor function score(FMA score)(P〉0.05). After treatment, the observation group had obviously higher FMA score as(45.10±3.09) points than(28.63±2.90) points in the control group, and the difference was statistically significant(P〈0.05). Before treatment, both groups had no statistically significant difference in modified Barthel index(MBI) score(P〉0.05). After treatment, the observation group had obviously higher MBI score as(79.63±9.31) points than(52.29±9.42) points in the control group, and the difference was statistically significant(P〈0.05). Before treatment, both groups had no statistically significant difference in visual analogue scale(VAS)(P〉0.05). After treatment, the observation group had obviously lower VAS score as(2.53±0.52) points than(5.48±0.68) points in the control group, and the difference was statistically significant(P〈0.05). Conclusion For patients with hemiplegic shoulder pain after stroke, combination of transcutaneous electrical nerve stimulation and shoulder girdle training help to further reduce the level of pain, and has a positive effect on improving the quality of life with better efficacy. It can be popularized.
