摘要
目的为临床肝脏手术和解剖学教学提供资料。方法 10例正常成人新鲜尸体肝脏标本,经肝固有动脉、肝门静脉和肝总管分别注入三种颜色自凝材料,用稀盐酸腐蚀肝组织,获得肝内管道铸型标本并测量相关数据。结果铸型标本对肝动脉、肝门静脉及肝胆管之间的解剖学关系显示清晰,肝动脉管腔最细,肝门静脉管腔最粗,三套管道的一、二级分支的排列关系有一定规律,到第三级分支以下相互间走行复杂,管腔很细且无明显规律。测量结果显示肝门静脉分叉角度为(107.3±6.8)°,肝固有动脉分叉角度为(81.2±7.1)°,肝总管分叉角度为(88.4±6.2)°。肝门静脉左支长度(23.3±2.4)mm,外径(7.7±1.0)mm;肝门静脉右支长度(18.9±3.2)mm,外径(11.0±1.5)mm;肝左动脉长度(26.0±2.3)mm,外径(2.9±0.5)mm;肝右动脉长度(25.5±2.5)mm,外径(3.2±0.7)mm;肝左管长度(14.6±2.3)mm,外径(4.7±0.6)mm;肝右管长度(9.2±2.4)mm,外径(4.5±0.4)mm。结论肝内管道的详细解剖可为肝脏手术中准确寻找和处理肝动脉、肝门静脉及肝胆管提供解剖学依据。
Objective: To provide information for clinical liver surgery and anatomy teaching. Methods: The hepatic arteries and hepatic portal veins and hepatic ducts of 10 cases of normal adult fresh cadaver liver specimens were injected 3 kinds of color self-curing material respectively. The liver tissue was stained with dilute hydrochloric acid to obtain intravascular piping specimens and the relevant data were measured. Results: The anatomical relationship of the hepatic arteries,hepatic portal veins and hepatic ducts were showed clearly in the casting mould specimens. The hepatic artery lumen is the thinnest and the hepatic portal vein lumen is thickest. The relationship is a certain law in one and two branches of the three sets of vessels. The lumen is very fine and no obvious laws from the third level of the branches. The results showed that the angle of hepatic portal vein bifurcation was( 107. 3 ± 6. 8) °,the angle of hepatic artery bifurcation was( 81. 2 ± 7. 1) °,and the angle of hepatic duct bifurcation was( 88. 4 ± 6. 2) °. The length of the left branch and the right branch of the portal vein was( 23. 3 ± 2. 4) mm and( 18. 9 ± 3. 2) mm,and the outer diameter was( 7. 7 ± 1. 0) mm and( 11. 0 ± 1. 5) mm respectively. The length of the left hepatic artery and the right hepatic artery was( 26. 0 ± 2. 3) mm and( 25. 5 ± 2. 5) mm,and the outer diameter was( 2. 9 ± 0. 5) mm and( 3. 2 ± 0. 7) mm respectively. The length of the left hepatic tube and the right hepatic tube was( 14. 6 ± 2. 3) mm and( 9. 2 ± 2. 4) mm,and the outer diameter was( 4. 7 ± 0. 6) mm and( 4. 5 ±0. 4) mm respectively. Conclusions: It can provide anatomical basis for accurate search and treatment of hepatic arteries,hepatic portal veins and hepatic ducts during liver surgery through the detailed anatomy of intrahepatic vessels.
