摘要
目的:研究低流量吸入地氟醚对患儿自身压力反射敏感性产生的影响。方法:取我院收治Ⅰ~Ⅱ级患儿66例作为研究对象,依据维持麻醉方式差异,分为A组(n=33)与B组(n=33),A组予以地氟醚,B组予以异氟醚,均实施低流量吸入法,对比两组各时间点心血管BRS、翻身后升至目标血压时间、复苏时间及术后不良反应产生情况。结果:同时刻,两组心血管BRS、术后一天内恶心呕吐率对比差异无统计学意义(P>0.05);组内对比:相比于T0,两组T_1、T_2、T_3及T_4时刻心血管BRS均显著减小(P<0.05);相比于T_1,两组T_2、T_3时刻心血管BRS均显著增加(P<0.05);相比于T_2,两组T_4时刻心血管BRS显著减小(P<0.05);相比于T_3,两组T_4时刻心血管BRS显著减小(P<0.05);A组翻身后升至目标血压时间、睁眼时间及指令伸舌时间均明显短于B组(P<0.05)。结论:对患儿以低流量吸入方式给予地氟醚与异氟醚,均能产生良好控制性降压效果,其中地氟醚可促进患儿快速复苏,安全性较高,具有积极应用价值。
Objective: To determine the effect of low-flow inhalation of Desflurane on self-baroreflex sensitivity in children. Methods:Sixty-six children with I- II level, who were hospitalized in our hospital, were included as the subjects in the study. According to the difference of anesthesia, all the patients were divided into group A (n= 33) and group B (n = 33). Group A group was treated with Desflurane, and group B was treated with Isoflurane, and both group received low-flow inhalation. The cardiovascular BRS, time to target blood pressure after turning over, recovery time and incidence of postoperative adverse reactions at different time points were compared between the two groups. Results.At the same time point, the differences in the cardiovascular BRS and incidence of nausea and vomiting within 1 day after the operation were not significant (P〉 0.05). Intra-group comparison :The cardiovascular BRS at T~, T=, Ta and T4 time points was significantly decreased compared with that at T0(P〈0.05). The cardiovascular BRS at T2 and T3 was significantly increased compared with that at Tx (P〈0.05). The cardiovascular BRS was significantly decreased at T4 compared with that at T2 (P〈0.05). The cardiovascular BRS was significantly decreased at T4 compared with that at T3(P〈0.05). The time to target blood pressure after turning over, time to eye opening and time to instructed tongue extending in group A were significantly shorter than those in group B (P〈0.05). Conclusion:Both of low-flow inhalation of Desflurane and Isoflurane can produce good controlled antihypertensive effect in children. Among them, Desflurane can promote the rapid recovery of children with high safety and positive application value.
