摘要
目的通过社区人群的横断面研究,探讨上海市奉贤区社区妇女更年期综合征的发生情况及影响因素。方法采用整群抽样方法于2016年4-6月随机抽取上海市奉贤区2个社区40~60岁的本市户籍妇女为研究对象,通过问卷调查获得研究对象的个人情况及更年期相关症状等信息,并采用Kupperman绝经指数量表评估。结果此次研究共纳入406名调查对象,发生更年期综合征169例(41.63%),其中轻度104例(61.54%)、中度62例(36.69%)、重度3例(1.78%)。前3位症状依次为疲劳179例(44.09%)、骨关节痛164例(40.39%)和失眠154例(37.93%)。多因素Logistic回归分析显示,影响更年期综合征的主要因素包括妇科良性疾病史、婚姻状况和工作状态。结论上海市奉贤区40~60岁妇女更年期综合征发病率较高,医疗卫生机构应针对妇女围绝经期的特点开展有针对性的更年期妇女保健服务。
Objective To explore the prevalence and influencing factors of menopausal syndrome among community women in Fengxian District of Shanghai by a cross-sectional study.Methods The 40-60-year-old women with registered permanent residence in Shanghai were selected from two communities in Fengxian District by cluster random sampling method from April to June in 2016,the personal information and menopausal symptoms were collected by a questionnaire,Kupperman menopausal index（KMI） was applied to evaluate the severity of menopausal syndrome.Results A total of 406 women were enrolled in the study finally,169 women（41.63%） were diagnosed as menopausal syndrome,including 104 women with mild menopausal syndrome（61.54%）,62 women with moderate menopausal syndrome（36.69%）,and 3 women with severe menopausal syndrome（1.78%）.The top three frequent symptoms were fatigue（44.09%,179 women）,joint pain（40.39%,164 women）,and insomnia（37.93%,154 women）.Multivariate logistic regression analysis showed that the main influencing factors of menopausal syndrome included medical history of benign gynecological diseases,marital status,and work status.Conclusion The prevalence rate of menopausal syndrome is high among the women aged 40-60 years old in Fengxian District,Shanghai.The medical and health institutions should develop menopausal maternal health care service targeting to the characteristics of perimenopause.
