摘要
目的评估不同血细胞压积(BCV)算法对儿童感染性疾病患儿校正全血C-反应蛋白(CRP)检测结果的影响。方法选取2016年5-6月华中科技大学同济医学院附属协和医院儿科住院治疗的390例感染性疾病患儿为观察组,另选取同期100例健康查体儿童为对照组。对照组采用特定蛋白分析仪测定血清CPR,观察1组采用实时动态BCV算法分析仪测定全血CRP值,观察2组采用固定红细胞比容(Hct)算法换算出全血CRP值。分析各观察组测得全血CRP与血清CRP结果的差异。结果 33%<Hct<53%时,两观察组校正后全血CRP水平与对照组血清CRP水平比较差异均无统计学意义(均P>0.05)。Hct<33%或Hct>53%时,观察1组与对照组CRP水平比较差异无统计学意义(P>0.05);观察2组与对照组CRP水平差异有统计学意义(P<0.05),观察组结果均高于对照组,且随着CRP水平增高结果偏差越大。结论实时动态BCV算法检测全血CRP准确率优于固定细胞压积算法。采用实时动态BCV算法测定全血CRP,与血清CRP结果有较好的一致性,结果更加准确可靠。尤其在血常规与CRP联合检测中,具有较高的准确性和便利性。
Objective To evaluate the effects of different blood cell volume(BCV) calculation methods on detection results of C-reactive protein(CRP) in whole blood of children with infectious diseases.Methods A total of 390 children with infectious diseases who were treated in Xiehe Hospital Affiliated to Tongji Medical College,Huazhong University of Science and Technology from May to June in 2016 were enrolled into this study as observation group,a total of 100 healthy children receiving physical examination in the hospital were selected as control group during the same period.Serum CPR levels of the children in control group were detected with specific protein analyzer,CPR levels in whole blood of the children in observation group 1 were observed by real-time dynamic BCV algorithm analyzer,and CPR levels in whole blood of the children in observation group 2 were observed using fixed red blood cell hematocrit algorithm.The results of whole blood CRP and serum CRP in different groups were analyzed.Results When hematocrit was between 33% and 53%,there was no statistically significant difference between whole blood CRP levels after the correction in observation group 1,observation group 2 and serum CRP level in control group(all P〈0.05).When hematocrit was less than 33% or more than 53%,there was no statistically significant difference in CRP level between observation group 1 and control group(P〈0.05),while there was statistically significant difference in CRP level between observation group 2 and control group(P〈0.05).The detection results of CRP in observation group 1 and observation group 2 were higher than that in control group,and the differences increased with increase of CRP level.Conclusion The accuracy of real-time dynamic BCV algorithm is significantly better than that of fixed red blood cell hematocrit algorithm.The consistency of results of whole blood CRP measured by real-time dynamic BCV algorithm and serum CRP is good,and the results are more accurate and reliable,especially in the com
