超声引导下膝关节手术个性化止血带压力选择的探讨 被引量:3

摘要 目的探讨采用超声引导找到膝关节手术止血带个性化压力选择的标准,找出无动脉血流出现的最佳止血带压力。方法将实施膝关节置换手术的患者100例随机分为试验组和对照组各50例,试验组依据超声技术观察腘动脉血流情况,以腘动脉血流无显像为最佳止血带压力。对照组采用传统方法,压力在收缩压的基础上加150 mm Hg。比较2组止血带充气前后心率、皮损情况、术后止血带部位肢体疼痛、止血效果及并发症发生率。结果 2组患者相比较,试验组充气压力(220.45±20.88)mm Hg显著低于对照组(360.51±18.26)mm Hg。试验组较对照组在止血带放气后心率变化平稳(P<0.05),皮损、肢体疼痛及并发症的发生率均显著降低,止血效果差异无统计学意义。结论采用超声引导检测腘动脉血流无显像可以获得较低止血带压力,使手术顺利进行,血流动力学稳定,能够减少止血带使用过程中的皮损、肢体疼痛、麻痹等并发症的发生。 Objective To explore the individualized pressure to stop bleeding in the knee surgery guided by ultrasound,and to find the best tourniquet pressure could stop the arterial blood flow. Methods 100 cases of patients with knee replacement were randomly divided into experimental group and control group,50 cases in each group. Observed the popliteal artery blood flow by ultrasonic technology to find the best tourniquet pressure in experimental group, in the control group used conventional method in contraction. The heart rate, skin lesions, the tourniquet part limb pain, hemostatic effect and complication rates in two groups were compared before and after the tourniquet inflatable. Results The inflation pressure of experimental group was (220.45 ± 20.88) mmHg vs. (360.51 ± 18.26) mmHg in control group (P 〈 0.05 ). Compared with the control group, the heart rate of the experimental group was stable after deflating (P 〈 0.05 ). Skin lesions, limbs pain and the incidence of complications were significantly reduced, there was no difference in statistically significant in hemostatic effect. Conclusion The excellent hemostatic effect can be achieved by ultrasound. In the experimental group,the suitable pressure can make the operation smoothly, hemodynamic stability, reduce in the process of using a tourniquet skin, body pain, paralysis and other complications
