摘要
目的研究白细胞和C-反应蛋白与临终患者深部念珠菌感染的关系。方法选取2013年6月-2016年6月医院收治的确诊为深部念珠菌感染的临终患者233例及同期住院患者200例作为研究对象,根据微生物检测结果分成四组,单纯细菌感染组100例、单纯念珠菌感染组101例、念珠菌联合细菌感染组132例和微生物阴性组100例,采用免疫透射比浊法测定C-反应蛋白水平,血细胞分析仪法测定白细胞,比较各组间C-反应蛋白与白细胞水平的差异。结果 233例深部念珠菌感染患者感染以白假丝酵母为主;单纯细菌感染组、单纯念珠菌感染组和念珠菌联合细菌感染组WBC、CRP水平均明显高于微生物阴性组,与单纯细菌感染组比,单纯念珠菌感染组和念珠菌联合细菌感染组WBC、CRP水平均明显升高(P<0.05);与单纯念珠菌感染组相比,念珠菌联合细菌感染组WBC、CRP水平均明显降低(P<0.05);治疗7d后,单纯念珠菌感染组和念珠菌联合细菌感染组WBC及CRP水平明显升高(P<0.05),与单纯念珠菌感染组相比,念珠菌联合细菌感染组WBC差异无统计学意义,CRP水平低于单纯念珠菌感染组(P<0.05);相关性分析结果显示,CRP水平与深部念珠菌感染程度呈正相关,WBC水平与临终患者深部念珠菌感染病情相关性不强。结论 C-反应蛋白水平与深部念珠菌感染病情程度呈正相关,可以用于念珠菌感染病情预测。
OBJECTIVE To study the correlation between the white blood cells and C-reactive protein（CRP）and deep Candida infection in dying patients.METHODS Totally 233 dying patients who were diagnosed with deep Candida infection and treated in the hospital from Jun 2013 to Jun 2016 were enrolled in the study,and 200 pa-tients who were hospitalized during the same period were recruited as the study objects.According to the result of microbiological detection,all of the enrolled study objects were divided into four groups,the single bacterial infec-tion group with 100 cases,the single Candida infection group with 101 cases,the Candida combined with bacteri-al infection group with 132 cases,the microorganism-negative group with 100 cases.The CRP level was deter-mined by using immunofluorescence turbidimetric method,the WBC counts were measured by hematology analy-zer,and the CRP level and WBC counts were compared among the four groups.RESULTS The patients with Can-dida albicans infection were dominant among the 233 patients with deep Candida infection.The levels of WBC and CRP of the single bacterial infection group,the single Candida infection group and the Candida combined with bacterial infection group were significantly higher than those of the microorganism-negative bacteria;the lev-els of CRP and WBC of the single Candida infection group and the Candida combined with bacterial infection group were significantly higher than those of the single bacterial infection group（P〈0.05）.The levels of WBC and CRP of the single Candida infection group and the Candida combined with bacterial infection group were sig-nificantly elevated after the treatment for 7 days（P〈0.05）,there was no significant difference in the WBC counts between the single Candida infection group and the Candida combined with bacterial infection group,the CRP level of the Candia combined with bacterial infection group was significantly lower than that of the single Candida infection group（P〈0.05）.The result of the correlation analysis indi
