摘要
目的研究益气活血复方合剂对表柔比星心脏毒性的预防效果。方法将接受表柔比星化疗方案的乳腺癌术后患者67例,随机分为观察组34例和对照组33例,对照组单纯应用表柔比星化疗,观察组在化疗基础上加服益气活血复方合剂。比较化疗前后患者心电图、肌酸激酶同工酶(CK-MB)、肌钙蛋白T(c Tn T)、脑钠肽(BNP)、左心射血分数(LVEF)、左室短轴缩短率(FS)的变化。结果化疗后,两组患者CK-MB、c Tn T及BNP水平均显著高于化疗前,且对照组患者升高程度更明显,观察组患者LVEF、FS指标均显著高于对照组患者,差异有统计学意义(P<0.05)。观察组患者心电图异常总发生率11.8%,显著低于对照组患者心电图异常总发生率36.4%,差异有统计学意义(P<0.05)。表柔比星积累量<100mg/m2时,两组患者心电图异常发生率比较,差异无统计学意义(P>0.05);表柔比星积累量100~600 mg/m2时,观察组患者心电图异常发生均显著低于对照组患者,差异有统计学意义(P<0.05)。结论益气活血复方合剂对化疗患者化疗药物的心脏毒性具有预防作用,效果显著。
Objective To study the preventive effect of Yiqi huoxue compound mixture on the heart toxicity induced by epirubicin.Methods 67 patients with breast cancer were randomly divided into two groups,34 in the observation group,and 33 in the control group. The control group was treated with epirubicin only,and the observation group was given the compound recipe of Yiqi huoxue compound mixture on the basis of chemotherapy.The ECG,CK-MB,c Tn T,BNP,LVEF and FS were compared before and after chemotherapy.Results The levels of CK-MB,c Tn T and BNP in both groups increased after treatment,but the increase of control group was more obvious,the LVEF and FS indexes in the observation group were significantly higher than those in the control group,the differences were statistically significant（ P〈0. 05）. The cumulative incidence of abnormal ECG in observation group was 11. 8 %,which was significantly lower than 36. 4% of control group（ P〈0. 05）. When epirubicin accumulation less than 100 mg/m^2,the difference of abnormal ECG in the two groups were not statistically significant（ P〈0. 05）,and when epirubicin accumulation between 100 and 600 mg/m^2,ECG abnormalities in the observation group was significantly lower than the control group, the difference was statistically significant（ P〈0.05）. Conclusions The preventive effect of Yiqi Huoxue compound mixture on the heart toxicity induced by epirubicin is remarkable.
