摘要
目的探讨糖尿病周围神经病变(DPN)对足底压力的影响。方法 113例糖尿病无周围神经病变患者[DPN(-)组]和121例DPN患者[DPN(+)组]采用Footscan 0.5米平板系统行震动感觉阈值(VPT)及足底压力检测,观察两组各年龄段患者左右足的VPT值、支撑时间、整足最大足底压力、平均足底压力、足部轴线。结果年龄<45岁患者:左足DNP(-)组的VPT值为(6.34±1.23)v,小于DPN(+)组的(10.47±6.46)v,差异有统计学意义(P<0.05);右足DNP(-)组的VPT值为(6.41±1.24)v,小于DPN(+)组的(9.53±5.55)v,差异有统计学意义(P<0.05);左右足两组支撑时间、整足最大足底压力、平均足底压力、足部轴线比较差异无统计学意义(P>0.05)。年龄45~59岁患者:左右足DNP(-)组的VPT值小于DPN(+)组,支撑时间低于DPN(+)组,差异有统计学意义(P<0.05);左足DNP(-)组平均足底压力高于DPN(+)组,差异有统计学意义(P<0.05);左足两组整足最大足底压力和足部轴线比较差异无统计学意义(P>0.05)。右足两组整足最大足底压力、平均足底压力、足部轴线比较差异无统计学意义(P>0.05)。年龄>59岁患者:左右足DNP(-)组的VPT值小于DPN(+)组,差异有统计学意义(P<0.05);左右足两组支撑时间、整足最大足底压力、平均足底压力、足部轴线比较差异无统计学意义(P>0.05)。VPT与年龄<45岁及年龄>59岁对应的左右足的足部轴线呈正相关(r=0.46、0.35,0.27、0.31;P<0.05),而与年龄45~59岁的足部轴线无相关性(P>0.05)。结论对糖尿病患者进行足部轴线的测定可进一步运用于糖尿病周围神经足溃疡风险的评估。建议对年龄45~59岁患者,首选支撑时间作为糖尿病足高危人群的筛查指标。
Objective To investigate the effect of diabetic peripheral neuropathy (DPN) on plantar pressure. Methods Footscan 0.5 m plate system was used to detect vibration perception threshold (VPT) and plantar pressure in 113 diabetic patients without diabetic peripheral neuropathy [ DPN (-) group ] and 121 DPN patients [ DPN (+) group ] . Their difference was statistically significant (P〈0.05). DNP(-) group had higher mean plantar pressure of left foot than DPN(+) group, and the difference was statistically significant (P〈0.05). Both groups had no statistically significant difference in maximum plantar pressure, mean plantar pressure and foot axis of the left feet (P〉0.05).
