摘要
目的探讨腹腔镜在闭合性腹部创伤患者中探查较开腹手术的优势。方法选取我院2014年6月至2017年6月收治的72例闭合性腹部创伤患者,随机分为腹腔镜组与开腹手术组各36例。腹腔镜组采用腹腔镜手术探查,开腹手术组采用开腹手术探查。比较两组患者的手术时间、术中出血量、术后排气时间、术后24 h VAS评分及术后并发症。结果腹腔镜组的手术时间、术中出血量、术后排气时间及术后24 h VAS评分均优于开腹手术组(P<0.05)。腹腔镜组的术后并发症发生率为5.6%,明显低于开腹手术组的27.8%(P<0.05)。结论与开腹手术比较,腹腔镜在闭合性腹部创伤患者中的探查具有手术时间短、术中出血少、术后疼痛小、恢复快等优势,还可明显降低术后并发症发生率,安全性及临床应用价值较高。
Objective To study the advantages of laparoscope compared with laparotomy in the exploration of patients with closed abdominal trauma. Methods 72 cases of patients with closed abdominal trauma admitted to our hospital from June 2014 to June 2017 were selected and randomly divided into laparoscope group and laparotomy group, with 36 cases in each group. The laparoscope group received laparoscopic exploration, while the laparotomy group received laparotomy exploration. The operation time, intraoperative blood loss, postoperative exhaust time, postoperative 24 h VAS score and postoperative complications were compared between two groups. Results The operation time, intraoperative blood loss, postoperative exhaust time and postoperative 24 h VAS score of laparoscope group were better than those of laparotomy group （P 〈0.05）. The incidence of postoperative complications of laparoscope group was 5.6%, significantly lower than 27.8% of laparotomy group （P〈0.05）. Conclusions Compared with laparotomy, laparoscope in the exploration of patients with closed abdominal trauma has advantages of shorter operation time, less intraoperative blood loss, less postoperative pain and quicker recovery, and can significantly reduce the incidence of postoperative complications, which has higher safety and clinical application value.
