神经生长因子对原发性闭角型青光眼的视神经保护作用 被引量:10

摘要 目的探讨神经生长因子(NGF)对原发性闭角型青光眼的视神经保护作用,为青光眼临床预防和治疗工作提供新的思路。方法采用前瞻性研究方法,选择原发性闭角型青光眼患者188例作为研究对象,根据随机平行对照原则分为观察组与对照组各94例,两组都给予小梁切除术治疗,观察组术后给予注射用鼠NGF治疗,对照组术后给予口服甲钴胺片治疗,用药观察时间为4周,观察预后视神经变化情况。结果治疗后观察组与对照组的视力都明显高于治疗前(P<0.05),同时治疗后观察组的视力也明显高于对照组(P<0.05)。观察组与对照组治疗后的眼压分别为(12.96±2.87)mmHg和(13.20±2.49)mmHg,都明显低于治疗前(P<0.05),不过在组间对比无明显差异(P>0.05)。观察组与对照组治疗后的P100振幅都明显高于治疗前(P<0.05),而P100潜伏期明显低于治疗前,组内对比差异明显(P<0.05),且观察组与对照组对比差异也有统计学意义(P<0.05)。两组治疗后的ET-1含量都明显低于治疗前(P<0.05),观察组治疗后的ET-1含量也低于对照组(P<0.05),两组NO含量组内与组间对比差异无统计学意义(P>0.05)。结论 NGF可促进原发性闭角型青光眼患者视力的改善,也有利于眼压的降低,可以促进视神经功能的恢复,可能与其调节血浆ET-1的表达有关。 Objective To investigate the protective effects of nerve growth factor (NGF) on the optic nerve in primary angle closure glaucoma. Methods A prospective analysis was conducted on 188 patients with primary angle clo- sure glaucoma in Tangshan eye hospital from August 2013 to February 2016. According to random parallel control princi- ple, the participants were randomly divided into observation group and control group ( n = 94 ). Trabecular resection were given in both 2 groups, while NGF injection and oral Mecobalamin treatment were given for 4 weeks in treatment and con- trol group, respectively. The optic nerve changes were observed afterward. Results After treatment, the visual acuities of treatment group and control group were both significantly higher than those before treatment ( P 〈 0. 05 ) , and the visual acuity of treatment group was significantly higher than that of control group ( P 〈 0. 05 ). The intraocular pressures in treat- ment group and control group after treatment were 12.96 ±2. 87 mmHg and 13.20 ± 2.49 mmHg, respectively, which were both significantly lower than those before treatment ( P 〈 0.05 ), but there was no significant difference between the 2 groups (P 〉 0. 05). The PI00 amplitudes in treatment group and control group after treatment were significantly higher than those before treatment, and P100 latencies were significantly lower than those before treatment, compared within groups differences were significantly (P 〈 0. 05 ), and there were significant differences in both P100 amplitude and laten- cy between treatment group and control group ( P 〈 0. 05 ). The levels of ET - 1 in the two groups after treatment were sig- nificantly lower than those before treatment, and it was significant lower in treatment group than control group ( P 〈 0. 05).
