髋关节置换围手术期深静脉血栓的防治 被引量:25

摘要 背景:一项比较物理联合药物与单独物理或药物预防深静脉血栓形成临床效果的荟萃分析显示,联合预防能够降低深静脉血栓形成发生的危险性。目的:分析髋关节置换围手术期深静脉血栓预防的有效措施。方法:采取前瞻性研究,将行髋关节置换后患者350例随机被分为2组,一组应用间歇充气加压装置与标准剂量低分子肝素干预(肝素联合IPC组);另一组只接受标准剂量低分子肝素干预(低分子肝素组)。观察两组病例在围手术期的深静脉血栓发生率。结果与结论:(1)肝素联合IPC组深静脉血栓形成发生率(11%),低分子肝素组深静脉血栓形成发生率(33%);肝素联合IPC组深静脉血栓形成发生率明显低于低分子肝素组(P<0.01);(2)两组患者均无症状性肺栓塞发生;(3)结果表明,应用间歇充气加压装置联合标准剂量低分子肝素抗凝防治深静脉血栓较单纯应用标准剂量低分子肝素效果更好。 BACKGROUND: A meta-analysis of the clinical effectiveness of physical therapy combined with drugs versussingle physical therapy or drugs for preventing deep venous thrombosis (DVT) has shown that combined therapycan effectively reduce the incidence of DVT.OBJECTIVE: To explore the effective measures of the prevention of perioperative DVT in hip arthroplasty.METHODS: This is a prospective study, 350 patients undergoing hip replacement were randomly divided into twogroups, followed by given the treatment of intermittent pneumatic compression combined with standard dose oflow-molecular-weight heparin (combination group), or only low-molecular-weight heparin (control group). Theperioperative incidence of DVT was observed.RESULTS AND CONCLUSION: (1) The incidence of DVT in the combination group was significantly lower than that inthe control group (11% vs. 33.1%, P 〈 0.01). (2) No symptomatic pulmonary embolism was found in both groups. (3) Toconclude, the anticoagulant prevention of DVT using the combination of intermittent pneumatic compression andstandard dose of low-molecular-weight heparin is more effective than the use of low-molecular-weight heparin alone.
