摘要
目的探讨复方中药制剂冠心Ⅴ号合剂治疗急性心肌梗死(AMI)对心室重构的影响及作用机制。方法选取2014年1月至2016年5月收治的AMI患者90例,按随机数字表法分为中药组和常规组,各45例。两组均采用口服阿司匹林肠溶片,皮下注射低分子肝素钙等基础治疗,中药组加用复方中药制剂冠心Ⅴ号合剂。两组均治疗8周。结果治疗8周后,中药组患者的血清氨基末端脑钠肽前体(NT-pro BNP)、超敏C反应蛋白(hs-CRP)、基质金属蛋白酶-9(MMP-9)、血管紧张素Ⅱ(AngⅡ)、内皮素-1(ET-1)水平,以及左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)、左室射血分数(LVEF)、左室收缩末期容积(LVESV)、左室舒张末期容积(LVEDV)、左室壁厚度(LVPWT)、室间隔厚度(IVST)均明显低于常规组(P<0.05),LVEF值明显高于常规组(P<0.05)。结论复方中药制剂冠心Ⅴ号治疗AMI,可通过降低血清中心室重构相关因子,发挥改善心功能、抑制心室重构的作用。
Objective To investigate the clinical effect of compound traditional Chinese medicine（TCM） preparation （Guanxin Ⅴ Mixture） in the treatment of ventricular remodeling after acute myocardial infarction （AMI） and its mechanism. Methods Totally 90 patients with AMI admitted to our hospital from January 2014 to May 2016 were selected and divided into the TCM group and the normal group according to the random number table method ,45 cases in each group. The two groups were treated with oral Aspirin Enteric - Coated Tablets, subcutaneous injection of low molecular weight heparin calcium and other basic treatment, on this basis, the TCM group was given Guanxin Ⅴ Mixture. The two groups were treated for 8 weeks. Results After 8 weeks of treatment, the levels of NT - proBNP, hs - CRP, MMP-9 ,Ang n and ET -1, LVEDD, LVESD, LVEF, LVESV, LVEDV, LVPWT, IVST of the TCM group were significantly lower than those of the normal group （P 〈 0.05）,but the level of LVEF was significantly higher than that of the normal group （P 〈 0.05）. Conclusion Compound TCM preparation （Guanxin Ⅴ Mixture） in the treatment of ventricular remodeling after AMI can improve heart function and inhibit ventricular remodeling by reducing the serum factors related to ventricular remodeling.
