摘要
目的探讨神经电生理检测在手麻、手臂酸痛中的诊断意义。方法选择2015年7月至2016年7月我院收治的手麻、手臂酸痛患者54例为研究对象,对其进行神经电生理检测,并将检测结果与MRI检查结果进行比较,以评价神经电生理检测在手麻、手臂酸痛中的诊断价值。结果 54例进行神经电生理检测的手麻、手臂酸痛患者中,50例患者检测异常,阳性率为92.59%。其中,神经根型颈椎病阳性率最高(33.33%),其次为神经根型颈椎病合并腕管综合征(27.78%)和腕管综合征(24.07%)。MRI检测腕管综合征、腕尺管综合征、肘管综合症与神经电生理检测结果一致。18例神经电生理检测为神经根型颈椎病的患者中,MRI检测13例异常,检测符合率为72.22%。结论神经电生理检测手麻、手臂酸痛的阳性率高,可为诊断提供依据,并有助于明确神经损伤的部位、范围及程度,鉴别诊断价值较高,不过该方法属于功能性检测,虽然具有较高的敏感性,但是特异性较低,因此临床诊断中应结合影像学检查,以提高诊断准确率,为临床治疗提供指导。
Objective To explore the diagnostic significance of electroneurophysiology detection in the diagnosis of hand numbness and arm sore. Methods Fifty-four cases of patients with hand numbness and arm pain in our hospital from July2015 to July 2016 were selected as the study objects, all the patients received electroneurophysiology detection, and the detection results were compared with that of MRI detection, so as to evaluate the diagnostic value of electroneurophysiology detection in the diagnosis of hand numbness and arm sore. Results Among the 54 hand numbness and arm sore patients who received electroneurophysiology detection, 50 cases were detected abnormal, the positive rate was 92.59%. The nerve root type of cervical spondylosis had the highest positive rate(33.33%), followed by nerve root type of cervical spondylosis combined with carpal tunnel syndrome(27.78%) and nerve root type of cervical spondylosis combined with carpal tunnel syndrome(24.07%). The electroneurophysiology detection results of carpal tunnel syndrome, carpal ulnar tunnel syndrome and cubital tunnel syndrome were consistent with the MRI detection results. In 18 nerve root type of cervical spondylosis diagnosed with electroneurophysiology detection, 13 cases were detected abnormal, the diagnostic accordance rate was72.22%. Conclusion The positive rate of lectroneurophysiology detection in the diagnosis of hand numbness and arm sore is high, it can provide an accurate basis for diagnosis, and help to clear the nerve injury location, extent, which has high differential diagnosis value. This detection method belongs to functional testing, although it has high sensitivity, but the specificity is low, so the clinical diagnosis should be combined with imaging examination to improve the diagnostic accuracy and provide guidance for clinical treatment.
