摘要
目的探讨叶酸、维生素B_(12)对幽门螺杆菌(Hp)阴性慢性萎缩性胃炎的疗效。方法选取我院Hp阴性慢性萎缩性胃炎患者67例,分为对照组和治疗组,对照组给予常规疗法(胃蛋白酶);治疗组给予常规疗法+叶酸、维生素B_(12),分别于0、12、24周对患者叶酸、维生素B_(12)、临床症状、胃镜及病理活检积分进行比较。结果治疗12周,两组患者胃镜、病理活检(活动性)积分及叶酸、维生素B_(12)水平比较,差异有统计学意义(P<0.05);临床症状、病理活检(慢性炎症、萎缩、肠化生)比较,差异无统计学意义(P>0.05)。治疗24周,两组临床症状、胃镜、病理活检(慢性炎症、活动性、萎缩)积分及叶酸、维生素B_(12)水平比较,差异有统计学意义(P<0.05);病理活检(肠化生)比较差异无统计学意义(P>0.05)。结论叶酸、维生素B_(12)能改善Hp阴性慢性萎缩性胃炎患者的临床症状及组织学变化,值得临床推广应用。
Objective To explore the therapeutic effects of the folic acid,vitamin B_(12) on Helicobacterpylori(Hp)-negative patients with chronic atrophic gastritis(CAG). Methods we enrolled 67 patients who werediagnosed as CAG of Hp-negative from the First Affiliated Hospital of Xi′an Medical University. The patients weredivided into control group and treatment group. Control group were given routine treatment(pepsin tablets),treatment group were given routine treatment and folic acid,vitamin B_(12). Then respective compared the folic acid,vitamin B_(12),clinical symptoms scores,gastroscopic scores and histopathological scores in the two groups beforetreatment and after treatment of 12 weeks and 24 weeks. Results After treatment of 12 weeks,there were signifi-cant differences in the gastroscopic scores and histopathological scores(activity),folic acid,vitamin B_(12) status(P < 0.05);no significant difference existed in clinical symptoms scores,histopathological scores(chronic inflam-mation,atrophic,intestinal metaplasia)between the two groups(P > 0.05). After treatment of 24 weeks,thedifferences of clinical symptoms scores,gastroscopic scores,histopathological scores(chronic inflammation,activity,atrophic,intestinal metaplasia) and folic acid,vitamin B_(12) status were significant(P < 0.05);nosignificant difference existed in histopathological scores(intestinal metaplasia) between the two groups(P >0.05). Conclusion Folic acid and vitamin B_(12) can improve the clinical symptoms and histological situation of the Hp-negative patients with CAG,worthy of further popularizing in clinic.
