液体分枝杆菌培养管与利福平耐药实时荧光定量核酸扩增检测技术检测结核杆菌的阳性检出率及检出时间比较 被引量:4

Comparisons on the positive detection rate and the detection time of using MGIT and XpertMTB/RIF in the test of Mycobacterium tuberculosis

摘要 目的比较采用液体分枝杆菌培养管(MGIT)培养法与利福平耐药实时荧光定量核酸扩增检测技术(XpertMTB/RIF)检测结核杆菌的阳性检出率及检出时间的差异。方法收集2016年7月-2017年4月本院就诊的疑似和治疗后复诊的肺结核患者痰标本共1 146例,其中疑似新发患者痰标本616例,治疗后复诊患者痰标本530例。分别采用液体MGIT培养法和XpertMTB/RIF检测法对所有痰标本进行检测,对2种方法的阳性检出率、检出时间及利福平耐药情况进行比较。结果疑似新发患者痰标本液体MGIT培养法阳性检出率明显低于XpertMTB/RIF检测法,差异有统计学意义(P<0.05);疑似新发患者痰标本液体MGIT培养法阳性检出时间大于XpertMTB/RIF检测法;治疗后复诊患者痰标本液体MGIT培养法阳性检出时间亦大于XpertMTB/RIF检测法,差异均有统计学意义(P<0.05)。结论 XpertMTB/RIF较MGIT能够提高结核杆菌的阳性检出率,缩短阳性检出时间,值得在临床检测中使用并推广。 Objective To compare the differences on the positive detection rate and the detection time using XGIT and XpertMTB/RIF in the test of Mycobacterium tuberculosis( MTB). Methods 1 146 sputum samples were collected from the suspected tuberculosis patients and confirmed ones for returned visit in our hospital from July 2016 to April 2017,including 616 suspected newly cases and 530 cases from follow-up patients. All cases were detected by liquid MGIT culture method and XpertMTB/RIF method,and the results were compared on positive detection rates,detection time and the resistance of RIF. Results The positive rate of suspected newly patients by liquid MGIT culture method was lower than that by XpertMTB/RIF,with the difference statistically significant( P〈0. 05); the detection time of suspected newly cases and the follow-up patients by liquid MGIT culture method was longer than those by XpertMTB/RIF,and there was no statistical significance on the differences( P〉0.05).Conclusion Compared to liquid MGIT culture method,XpertMTB/RIF could increase the positive detection rate of MTB and reduce detection time,which is worthy of clinical diagnosis promotion and use.
