摘要
目的评价痰和支气管肺泡灌洗液(BALF)两种标本的结核杆菌培养(TB培养)的检测结果在肺结核诊断中的临床价值。方法回顾分析南京市胸科医院2016年5月到2017年4月收治的1085例住院患者标本,其中肺结核组1000例,非肺结核组85例,收集所有患者的痰和BALF标本TB培养的检测结果,比较两种不同标本培养的灵敏度差异和性能指标。结果 (1)痰和BALF的结核分枝杆菌培养灵敏度分别为35.70%和36.30%,诊断效率分别为40.74%、41.29%,肺结核组两种标本TB培养结果比较差异无统计学意义(P>0.01)。(2)两种标本联合检测在肺结核诊断中的灵敏度为45.10%,诊断效率为49.40%,与痰和BALF分别检测相比,差异均有统计学意义(P<0.01)。结论痰和BALF两种标本的结核分枝杆菌培养在肺结核的诊断上没有差异,两种标本联合检测能更有效的提高肺结核诊断率。
Objective To evaluate the clinical value of detection of mycobacterium tuberculosis culture (TBculture ) in sputum and bronchoalveolar lavage fluid ( BALF) in the diagnosis of pulmonary tuberculosis. Methods A total of 1,085 patients who were initially diagnosed were collected from May 2016 to April 2017,inses were the tuberculosis group,and the other 85 cases were the non-tuberculosis group. Their sputum and BALF specimens were collected fromall patients,and TB culture were performed in all specimens. The sensitivity of the two different specimens was compared. Results 1. The TB culture sensitivity of sputum and BALF was 35. 70 % and 36. 30 % respectively,and the accuracy was 40. 74 % and 41. 29% respectively. There was no significant differencein TBculture between the two groups (P〉 0. 01 ). 2. The sensitivity of combined detection of diagnosis of pulmonary tuberculosis was 45. 10 %,and the accuracy was 49. 40 % . Compared with sputum and BALF,the difference was statistically significant (P〈 0. 01) . Conclusion There is no diference in TB culture be-tween sputum and BALF in the diagnosis of pulmonary tuberculosis. The combined detection of tlie two specimens can improve the diagnosis rate of pulmonary tuberculosis more effectively.
