摘要
目的探讨胸锁乳突肌瓣在预防腮腺区良性肿瘤术后畸形及味觉出汗综合征中的临床应用。方法2010年1月至2014年12月共56例腮腺良性肿瘤患者,行腮腺肿瘤及浅叶区域腺体或全部浅叶切除术。有21例一期行胸锁乳突肌瓣转移修复腮腺区,其他35例未行任何组织瓣修复。术后随访3月至24月,观察面部腮腺术区外形情况,并行客观试验检查。结果一期采用胸锁乳突肌瓣修复者的面部外形基本对称,21例中出现味觉出汗综合征的2例;未行任何组织瓣修复者的腮腺术区均有不同程度的凹陷畸形,35例中出现味觉出汗综合征的18例,两者间存在显著性差异(P<0.05)。结论胸锁乳突肌瓣修复腮腺良性肿瘤术后畸形及预防味觉出汗综合征等方面,切实有效,值得临床上应用。
Objective To access the effect of sternocleidomastoid muscle flap in the restoration of sunken deformity of face, and preventing Frey＇s syndrome after parotidectomy.Methods In this study,56 patients of parotid benign tumor underwent part superfcial lobectomy, and soft tissue defects of 21 patients were repaired by the sternocleidomastoid muscle flap , the soft tissue defects of other 35 patients were not repaired by any fap. From 3 months to 24 months after operation,the sunken deformity of faces was observed, and all patients were examined by iodine starch test.Results There were obvious sunken deformity of faces in group B, and 30 in 93patients were positive in iodine starch test, with the rate of 32.3%. While there were no obvious sunken deformity of faces in group A, and only 7 in 48 patients were positive in iodine starch test,with the rate of 14.6%. The positive rate of iodine starch test were signifcantly lower in.Conclusion Sternocleidomastoid muscle fap is effective to correct the deformity of face after parotidectomy and decrease the occurence of Frey＇s syndrome.
