摘要
目的观察中医药维持治疗广泛期小细胞肺癌的治疗效果。方法采用前瞻性队列研究设计,纳入一线治疗后达到缓解或稳定的广泛期小细胞肺癌患者60例,按接受中医治疗与否分为中医维持治疗组30例和对照组30例,分别进行临床观察、定期随访,直至疾病进展。观察两组患者无进展生存期(PFS)、体力状况(KPS评分)及中医证候疗效。结果可评价病例共57例,其中中医维持治疗组28例,对照组29例。中医维持治疗组治疗后平均PFS与对照组比较差异无统计学意义(P>0.05)。中医维持治疗组患者经治疗2个月后体力状况改善程度优于对照组,差异具有统计学意义(P<0.05)。中医维持治疗组患者经治疗1、2个月后中医证候疗效优于对照组,差异具有统计学意义(P<0.05)。结论广泛期SCLC患者一线化疗后进行中医药维持治疗未能延长患者的无进展生存时间,但能改善患者的体力状况、减轻临床症状,提高生活质量。
Objective To observe the therapeutic effects on small cell lung cancer at extensive stage in the maintenance treatment with Chinese herbal medicine. Methods The prospective cohort trial was a- dopted. 60 patients of small cell lung cancer at extensive stage were collected in compliance with the relieved or stable condition after the first - line chemotherapy. The treatment group （ 30 cases ） and the control group （30 cases）were divided according to whether TCM maintenance was applied or not. The clinical observation and regular follow - up visit were conducted till the disease was progressed. The research index included the progression- free- survival span（PFS）, physical condition （KPS score）and the therapeutic effects of TCM syndrome. Results Totally 57 cases were included in the evaluation, in which, 28 cases were in the treat- ment group with TCM maintenance and 29 cases in the control group. The average PFS was not different sta- tistically in comparison of the two groups（ P 〉 0.05 ）. In 2 months of TCM maintenance, the physical condi- tion improvements in the treatment group were better significantly than those in the control group, indicating the statistical significance （ P 〈 0.05 ）. In 1 and 2 months of TCM maintenance, the therapeutic effects on TCM syndromes in the treatment group were better significantly than those in the control group,indicating the statistical significance（P 〈 O. 05 ）. Conclusion The maintenance treatment with Chinese herbal medicine after the first - line chemotherapy cannot prolong PFS in the patients of small cell lung cancer at extensive stage, but it improves the physical condition, alleviates the clinical symptoms and improves the living quality.
