摘要
目的探讨早期护理干预对新生儿缺氧缺血性脑病的效果。方法选择2013年1月~2015年12月在我院治疗的HIE患儿60例为研究对象,均为中度HIE。将60例患儿随机分为干预组与对照组各30例。对照组给予常规治疗及护理,干预组给予常规治疗及早期护理干预。分别于入院时、日龄14 d以及日龄28 d时对患儿进行新生儿神经行为评定(NBNA);分别于3个月龄、6个月龄对患儿采用婴幼儿智能发育量表CDCC进行智力发育指数以及心理运动发育指数评价。在1岁时采用Gesell发育量表对患儿智能发育情况进行评价。结果两组14 d及28 d NBNA评分均较入院时显著升高,差异有统计学意义(P<0.05);干预组14 d及28 d NBNA评分均显著高于对照组,差异有统计学意义(P<0.05)。两组6个月龄MDI、PDI评分较3个月龄均有提高,差异有统计学意义(P<0.05)。干预组3个月龄及6个月龄PDI、MDI评分均显著高于对照组,差异有统计学意义(P<0.05)。Gesell发育诊断量表评分结果显示:干预组在适应性能区、大运动能区、细动作能区、语言能区以及社交能区得分均显著高于对照组,差异有统计学意义(P<0.05)。结论新生儿缺氧缺血性脑病早期给予护理干预,能够显著改善患儿神经智能发育。
Objective To explore the effect of early nursing intervention on neonatal hypoxic-ischemic encephalopathy.Methods A total of 60 children patients with moderate HIE who were treated in our hospital from January 2013 to December 2015 were selected as the research subjects. 60 children patients were randomly divided into intervention group and control group, with 30 patients in each group. The control group was given routine treatment and nursing care, and the intervention group was given routine treatment and early nursing intervention. The neonatal neurobehavioral assessment(NBNA) was performed for the children patients at the time of admission, at 14 days of age and 28 days of age; the mental development index and psychomotor development index were evaluated by infants' intelligence development scale CDCC at 3 months and 6 months of age. At the age of 1 year old, the Gesell development scale was used to evaluate the intelligence development of children. Results The NBNA scores in the two groups were significantly higher than those at 14 days and 28 days after admission, and the differences were statistically significant(P〈0.05); the NBNA scores in the intervention group were significantly higher than those in the control group at 14 and 28 days, and the differences were statistically significant(P〈0.05). The MDI and PDI scores in the two groups at 6 months of age were significantly higher than those at 3 months of age, and the differences were statistically significant(P〈0.05). The PDI and MDI scores in the intervention group were significantly higher than those in the control group at 3 months and 6 months of age, and the differences were statistically significant(P〈0.05). The scores of the Gesell developmental diagnostic scale showed that the scores of the adaptive exercise area, large motor energy area, fine action area, language and social energy area were significantly higher in the intervention group than those in the control group, and the differences were statistically signif
