摘要
目的探讨血浆脂蛋白[lipoprotein(a),Lp(a)]浓度与大动脉功能和结构相关性。方法入选372例既往无明确心肌梗死和脑梗死病史门诊患者,收集患者人口学资料以及实验室指标;检测脉搏波传导速度(pulse wave velocity,PWV)和颈动脉内-中膜厚度(carotid intima-media thickness,CIMT)。根据空腹血浆Lp(a)浓度,将患者分为两组:<30 mg/d L(n=192)组和≥30 mg/d L(n=180)组,进行相关指标组间比较。结果 (1)血浆Lp(a)浓度与吸烟、糖尿病、高脂血症、低密度脂蛋白胆固醇和血肌酐浓度呈正相关,与降糖治疗以及他汀类药物治疗呈负相关。(2)多重线性回归分析提示吸烟、收缩压、原发性高血压(高血压)、糖尿病、高脂血症、Lp(a)浓度均与PWV和CIMT呈正相关,而降压治疗、降糖治疗和他汀类药物使用与PWV和CIMT呈负相关。(3)与Lp(a)≥30 mg/d L组相比,Lp(a)<30 mg/d L组CIMT较小[(0.92±0.23)mm vs.(1.22±0.34)mm,P=0.017)];PWV也较小[(9.4±0.6)m/s vs.(12.4±0.8)m/s,P=0.026],差异有统计学意义。两组在其他变量方面比较,差异无统计学意义(P>0.05)。结论对于既往无明确心肌梗死和脑梗死患者,血浆Lp(a)浓度升高与PWV加快和CIMT增加具有一定的相关性。
s:Objectives To explore the relationship of plasma concentration of[lipoprotein(a),Lp(a)]with function and structure of main artery. Methods Totally 372 outpatients without obvious myocardial infarction and cerebral infarction medical history were included in this study. Demographic data and laboratory indexes were recorded. Pulse wave velocity(PWV)and carotid intima-media thickness(CIMT)were measured. According to plasma concentration of Lp(a),patients were divided into two groups:〈30 mg/dL group(n=192)and≥30 mg/dLgroup(n=180). Indexes between the two groups were compared. Results (1)Plasma concentration of Lp(a)positively correlated with smoking,diabetes,hyperlipoidemia ,low density lipoprotein cholesterol and serum creatinine;it negativly correlated with hypoglycemic therapy and statins therapy.(2)Multiple linear regression analysis indicated that smoking ,systolic blood pressure,essential hypertension,diabetes,hyperlipoidemia,concentration of Lp(a)positively correlated with PWV and CIMT;however,hypoglycemic therapy,hypoglycemic therapy,statins therapy negativly correlated with PWV and CIMT.(3)Compared with Lp(a)≥30 mg/dL group,CIMT[(0.92±0.23)mm vs.(1.22±0.34)mm,P=0.017)]and PWV[(9.4±0.6)m/s vs.(12.4±0.8)m/s,P=0.026]in Lp(a)〈 30 mg/dL group were significantly lower. Conclusions In patients without obvious myocardial infarction and cerebral infarction medical history ,plasma concen?tration of Lp(a)elevating correlated with PWV accelerating and CIMT increasing.
