摘要
目的对引流管拔除困难的常见原因进行总结,采用拔管前局部应用灭菌石蜡油联合利多卡因胶浆局部处理后拔管与传统拔管方式进行比较。方法将42例腹部手术患者随机分为2组。对照组常规消毒,拆除固定后拔管;试验组应用石蜡油联合利多卡因胶浆预处理后拔出引流管。应用疼痛量表评估两组患者的疼痛等级,比较两组医师一次拔管成功率及拔管相关并发症发生率。结果与对照组比较,试验组因拔管刺激导致的疼痛明显缓解(P<0.05);试验组医师一次拔管成功率明显提高(P<0.05),两组拔管相关并发症发生率比较差异无统计学意义(P>0.05)。结论应用石蜡油联合利多卡因胶浆拔管具有取材简单、操作方便、应用安全、效果确切的特点,既减轻了患者的机体痛苦,又不增加患者的经济负担,值得基层医疗单位推广。
Objective To summarize the risk factors of difficulty in drainage tube removal, and compare the effects of extraction between local use of liquid paraffin and lidocaine mucilage before extubation and traditional extubation methods. Methods Totally 42 patients undergoing abdominal surgery were randomly divided into 2 groups. Patients in control group received routine anti-infection, and the drainage tube was removed after stitches were taken out,while patients in experiment group were given pretreatment of liquid paraffin combined with lidocaine mucilage to remove the drainage tube. Pain assessment scale was recorded,once extubation ratio and complication incidence were compared between the two groups. Results Compared with control group, the pain indured by extubation in experiment group was alleviated more obviously（ P〈0. 05）, the once extubation ratio was improved（ P〈0. 05）,while there was no significant difference in the incidence of complications between the two groups（ P〉0. 05）. Conclusion Extubation of liquid paraffin combined with lidocaine mucilage is simple, convenient, safe and effective. It can alleviate the sufferings of patients without increasing the financial burden. It has a good application prospect in primary hospital.
