摘要
目的 分析经皮单侧椎体成形术治疗骨质疏松性椎体压缩骨折的疗效.方法 选择骨质疏松性椎体压缩骨折患者90例（113椎）为研究对象,均行经皮单侧椎体成形微创手术,术后随访时间6个月,分析术后VAS疼痛评分、Cobb角以及不良反应等.结果 90例患者手术均成功完成,术后7 d及30 d的VAS评分分别为（2.02±0.11）分及（1.11±0.36）分,明显低于治疗前的（5.38±1.72）分,差异均有统计学意义（t=18.494、23.051,均P〈0.05）;术后7 d及30 d复查Cobb角,分别为（3.28±1.88）°及（3.57±1.63）°,与术前差异均有统计学意义（t=83.808、87.429,均P=0.000）;术后7 d及6个月之间Cobb角差异无统计学意义（P〉0.05）;术后渗漏骨水泥15例,占16.67%;术后死亡5例,包括2例肿瘤,1例心源性疾病,2例脑血管相关疾病;术后6个月复查X线正侧位片,结果提示相邻椎体发生压缩性骨折1例,占1.11%.结论 骨质疏松性椎体压缩骨折患者经术者手法复位后,施行单侧椎体成形微创术治疗方案,缓解疼痛效果良好,还可使部分骨折恢复正常高度,降低椎体不稳定风险.
Objective To analyze the efficacy of transcutaneous unilateral vertebroplasty in the treatment of osteoporotic vertebral compression fractures. Methods 90 cases （ 113 vertebrae ） with osteoporotic vertebral compression fractures were treated, and the patients were treated with minimally invasive percutaneous unilateral vertebroplasty. The follow - up time was 6 months, and the changes of VAS pain score, Cobb angle and adverse reactions were analyzed. Results The VAS scores of postoperative 7d and 30d were （ 2. 02 ± 0. 11 ） points and （1.11±0.36）points,respectively,whichweresignificantlylowerthanbeforetreatment[（5.38±1.72）points,t=18. 494 and 23. 051,all P〈0. 05]. The Cobb angles of postoperative 7d and 30d were （3. 28 ± 1. 88）°and （3. 57 ± 1.63）°,respectively,the differences were statistically significant compared with before treatment （t = 83. 808, 87. 429,all P=0. 000）. The Cobb angle between postoperative 7 days and postoperative 6 months had no statistically significant difference （P〉0. 05）. There were 15 cases （16. 67%） of postoperative leakage of bone cement,5 cases died after operation,including 2 cases of pulmonary embolism,1 case of cardiogenic diseases,2 cases of cerebrovascular related diseases. 6 months after surgery, review X - ray was lateral radiographs, the results showed that adjacent vertebral compression fractures in 1 case, accounting for 1. 11%. Conclusion The patients with osteoporotic compression fractures treated by unilateral vertebroplasty minimally invasive surgery can effectively relieve pain and cause some vertebral bodies to return to normal height and reduce the risk of vertebral instability.
