摘要
目的:通过对踪近事件重点纳入体系管理,加强踪近事件上报率,减少重复不良事件的发生,促进医疗质量持续改进。方法:对样本医院2016年1?12月在不良事件上报平台累计抓取踪近事件1409例进行研究分析。结果:医疗处置治疗事件有872例,占61.89%;检查/检验事件有297例,占21.08%。上报踪近事件最多的科室是儿科系统,有463例,占32.86%;其次是内科系统,有447例,占31.72%,两个系统占到踪近事件上报的64.58%。上报人员主要为工作5年以内,初级职称人员,医务人员多于护理人员。踪近事件中最主要的原因为工作人员因素,有1034例,占73.39%;第2位为沟通因素,有153例,占10.86%;第3位是流程因素,有121例,占8.59%。结论:踪近事件管理将是未来不良事件管理的新趋势,是医疗机构可持续改进服务质量的新要求,也是医院安全文化有效落地的重要体现。
Objective： To strengthen the trace recent events reported rate, reduce duplication of the occurrence of adverse events, and promote medical quality continuous improvement through the trace near event key into system management. Methods： Total 1409 cases of trace events in January - December 2016 in adverse event reporting platform had been captured and analyzed. Results： Medical treatment to treat events accounted for 872 cases, 61.89% of total trace recent events. The second is to check/test events in 297 cases, accounting for 21.08%. Report to trace is one of the most recent events department of pediatrics system of 463 cases （32.86%）, followed by 447 cases of internal system, accounted for 31.72%, the two systems to trace recent events reported 64.58%. The researchers report, mainly work for 5 years, primary title, medical personnel more than the nursing staff. Trace recent events is the main reason for staff factors in 1034 cases, accounting for 73.39%, the second for communication factors in 153 cases, accounting for 10.86%, the third is the process factors of 121 cases, 8.59%. Conclusion： Trace recent event management will be the new trend of future adverse events management, also the new requirement of medical institutions of sustainable improvement service quality and the important embodiment of hospital safety culture implementation.
