摘要
目的探讨高危妊娠孕产妇抑郁、育儿胜任感和社会支持从妊娠晚期至产后3-5天的变化及其相关性。方法采用爱丁堡产后抑郁量表、中文版育儿胜任感量表、领悟社会支持量表对180名高危妊娠孕产妇于妊娠晚期及产后3-5天进行调查。结果高危妊娠孕产妇妊娠晚期抑郁得分为(8.29±4.17)分,育儿胜任感量表总分为(74.14±9.39)分,领悟社会支持得分为(61.83±9.56)分;产后早期抑郁得分为(8.95±5.29)分,育儿胜任感量表总分为(76.44±10.34)分,领悟社会支持总分为(64.47±8.01)分;高危孕产妇妊娠晚期与产后3-5天育儿胜任感及社会支持得分差异有统计学意义(P<0.05),抑郁得分差异无统计学意义(P=0.20)。高危妊娠孕产妇妊娠晚期与产后早期抑郁、育儿胜任感和社会支持均存在不同程度相关性(r=-0.490~0.774,P<0.05)。结论高危妊娠孕产妇妊娠晚期以及产后抑郁症状及母亲角色适应有待改善,医护工作者应鼓励家庭参与育儿活动,提高孕产妇社会支持水平,进而促进孕产妇母亲角色适应,改善产后抑郁状况。
Objective：To investigate the variation trend of among depression,parenting sense of competence and social support in mothers with high-risk pregnancy from later trimester of pregnancy to early postpartum（3-5 day）. Methods：One hundred and eighty pregnant mothers with high-risks in later trimester of pregnancy and early postpartum were investigated via questionnaires including Edinburgh Postnatal Depression Scale（EPDS）,Parenting Sense of Competence Scale（PSOC）and Perceived Social Support Scale（PSSS）. Results：The mother with high-risk in the later trimester of pregnancy got 8.29±4.17 in EPDS,74.14±9.39 in PSOC and 61.83±9.56 in PSSS and in early postpartum got 8.95±5.29 in EPDS,76.44±10.34 in PSOC and 64.47±8.01 in PSSS. The scores of PSOC and PSSS were significantly different during these two periods,while the scores of EPDS weren′t. Depression,parenting sense of competence and social support have negative correlation relationship during later trimester of pregnancy and early postpartum（r=-0.490-0.774,P〈0.05）. Conclusion：Depression and parenting sense of competence in mothers with high-risks still need to be improved. Nurses should encourage families to participate in parental activities and improve maternal social support level,thus promote maternal role adaptation and depression.
